| Objective:To observe the effect of lensectomy on the patients withvarying degrees of angle closure and to provide clinical guidance for glaucomawith cataract surgery.Methods:39cases (45eyes) with varying degrees of angle closure andcataract were chosen and divided into three groups according to different statesof angle closure:A:9cases (12eyes), narrow angle with limited adhesionwithout defect of visual field and angle adhesion closed≤180°. B:19cases (22eyes), angle closure with defect of visual field,180°<angle adhesion closed≤270°. C:11cases (11eyes), continuing status primary angle closure, unseencontinuing status. Phacoemulsification with PC-IOL implantation andgoniosynechialysis were performed on patients from three groups. All patientswere follow-up for one to three months. The postoperative visual acuity,intraocular pressure, anterior chamber depth, angle and complications wereanalyzed.Results:Group A: the preoperative average intraocular pressure was21.28±6.23mmHg, while average intraocular pressure was19.0±2.83mmHgadhesion of angle was reopened after surgery. Anterior chamber depth was2.46±0.4mm before surgery and changed to3.54±0.43mm after surgery. Group B:the preoperative average intraocular pressure was21.22±7.52mmHg, while average intraocular pressure was16.0±3.80mmHg and adhesion of angle wasangle adhesion closed≤180°after surgery. Anterior chamber depth was2.01±0.44mm before surgery and changed to3.21±0.48mm after surgery. Group C:the preoperative average intraocular pressure was41.27±11.82mmHg, whileaverage intraocular pressure was16.96±5.32mmHg and adhesion of anglewas re-openedafter surgery. Anterior chamber depth was2.46±0.4mm beforesurgeryand chagned to2.46±0.4mm after surgery. All patients had deeperanterior chamber depth and enhanced visual acuity after surgery.Conclusions:1. To the patients with varying degrees of angle closure,adhesion of angle were re-opened after Phacoemulsification with PC-IOLimplantation and goniosynechialysis.2. To the paients with primary closedangle glaucoma with cataract, Phacoemulsification with PC-IOL implantationand goniosynechialysis was one of treatments to be chosen, which was able tocontrol intraocular pressure effeciently, re-open anterior chamber angle anddeepen the anterior chamber after surgery. |
