Objective:This study aimed at seeking more effective methods that will provide basis for the prevention of medication errors by using the evidence-based method, and finding a suitable and effectively method of reducing medication errors for our courtry.Methods:The study is to constitute a preliminary prevention strategies of reducing medication errors by using the evidence-based method, And using the Delphi method table entries within the localization. Eventually come to prevention strategies of reducing medication errors of Zhejiang. The Enthusiasm Coefficient of experts, the Authority Coefficient of experts, the Coefficients of Vatiation and the Harmonization Coefficient W were used to examine the reliability and representation of expert consultation.Results:Constructed four parts of37prevention strategies to reduce medication errors containing prescribing, dispensing, administration, management. After2rounds of expert consultation, the Enthusiasm Coefficient of experts was88.89%and100.0%(>60%). The Authority Coefficient of experts were both0.854(>0.7). The Coefficients of variation of the importance of all items ranged from0.00to0.21, those of operability of all items ranged from0.04to0.30.Conclusions:Constructed four parts of37prevention strategies to reduce medication errors containing prescribing, dispensing, administration, management. It also established evidence level and quality of each strategy. The reliability of the2rounds expert consultation showed that the expert consultations had good reliability and representation.
