Objective The current study investigated the clinical application of vancomycins in a3-A grade hospital from2012-2013to evaluate its rationality of use and establish an HPLC method to determine the concentration of vancomycins in human plasma, and use the method in critically ill patients in order to provide reference for clinical application.Methods We selected the patients treated with vancomycins injection in a3-A grade hospital from2012-2013. Clinical data were collected by electronic medical record (EMR) and hospital information system (HIS). The medicines directions and relevant guidelines for diagnosis and treatment of antibacterial were used for reference to evaluate the rationality including clinical efficacy and safety of vancomycins injection application. An HPLC method was establish to determine the concentration of vancomycins in human plasma, and use the method in critically ill patients.Results During212patients, five bacteria were detected, including119strains of Staphylococcus,28strains of E.coli,23strains of Klebsiella pneumoniae,21strains of Pseudomonas aeruginosa and21strains of Acinetobacter. Among119strains of Staphylococcus, there were39strains of Staphylococcus aureus,33strains of Staphylococcus epidermidis,25strains of Methicillin resistant Staphylococcus aureus and22strains of Methicillin resistant Staphylococcus aureus. Five bacteria showed none tolerance on vancomycins. The adverse effect rate of vancomycins was14.62percent. DUI value of vancomycins was less than1, the average days of drug administration were less than10days. The all-in cost of vancomycins was667296, and every person was3147.62, average drug use was8.37days, and effective rate was81.40percent.Conclusion There is some irrational clinical application of vancomycins injection in the3-A grade hospital, and which indicate that there is space for standardized vancomycins injection application to develop. Research shows that it has high rate of adverse effect on platelet, which should be confirmed in future practice. The method is simple, sensitive, accurate and suitable for the determination of vancomycins in critical ill patients plasma and clinical pharmacokinetic studies. |
