Risk Factors Of Type 2 Diabetes Mellitus Combined With Complications Such As Hepatic Fibrosis

Objective:To investigate the clinical risk factors of type 2 diabetes mellitus (T2DM) combined with hepatic fibrosis and microvascular complication.Methods:Totally 112 patients with T2DM were selected in our study and divided into 2 groups according to the Fibroscan measured:group A, T2DM combined with hepatic fibrosis (n=44); group B, T2DM without combined with hepatic fibrosis (n=68). Then 112 patients were divided into 2 groups according to microvascular complication:group A’, T2DM combined with microvascular complication; group B’, T2DM without combined with microvascular complication. The laboratory examination were collected and analyzed with univariate analysis and multivariate analysis.Results:(1)Patients of group A had a significantly higher ratio of having hypertension history, fatty liver, poor blood glucose control, BMI, ALT, AST than those in group B (χ2 =7.664,7.316,4.589; t=-3.121, Z=-2.121,-2.821; P<0.05). White blood cell count, neutrophile count, MCV in group A were significantly lower than those in group B (t=2.740,2.789,2.876; P<0.01). Patients of group A’had a significantly higher of age, course of diabetes, globulin, creatinine, the percent of neutrophile granulocyte, nertrophil count, rate of hypertension than those in group B’(P<0.05), while lymphocytes percentage, GFR, triglyceride in group A’were significantly lower than those in group B’ (P<0.05). (2)Logistic regression analysis showed that hypertension history, fatty liver, increased AST, decreased white blood cell count and decreased MCV were the independent risk factors of T2DM combined with hepatic fibrosis(OR=3.576,3.850,1.110, 0.669,0.808, P<0.05). Age, course of diabetes and nertrophil count were the independent risk factors of microvascular complication in patients with type 2 diabetes(OR=1.155, 2.145,2.275, P<0.01).Conclusion:Hypertension history, fatty liver, increased AST, decreased white blood cell count and decreased MCV were the related risk factors of T2DM combined with hepatic fibrosis. Subclinical inflammatory was correlated to microvascular complication in patients with type 2 diabetes. This study provides a theoretical basis to distinguish patients with hepatic fibrosis and microvascular complication in T2DM.
