The Predictive Value Of NT-proBNP And IL-6 In Low-moderate Risk Troponin-negative Non-ST Elevation Acute Coronary Syndrome Patients

Aim:To detect the peripheral blood levels of Amino-terminal Pro-B-type Natriuretic Peptide(NT-proBNP) and Interleukin-6(IL-6) in low-moderate risk troponin-negative Non-ST elevation acute coronary syndrome(NSTE-ACS) patients,clinical follow up at 6 months was performed for adverse endpoints.To study whether these biomarkers could be predictors of low-moderate risk troponin-negative NSTE-ACS patients in short-term prognosis.Methods: According to patients’ history, symptoms, biomarkers, ECG and coronary arterigraphy,totally 106 patients were enrolled.We collected these patients’ blood samples in 24 hours after admission. The peripheral blood levels of NT-proBNP and IL-6 were measured with enzyme-linked immunosorbent assay(ELISA). All patients were followed up during 6 months. Cardiovasular events during observation were recorded. According their endpoints we divided these patients into two groups. The incidence of cardiovascular events in different groups were compared.Results: 1.Through the calculation of the SPSS software, according to the ROC curve,we select a better cut-off point as diagnostic reference values. We selected ROC curve to analysis results, it indicated that serum markers to determine the composite outcome efficiency, the best cutoff value(cutoff value) were that NT-proBNP is 512.62pg/ml and IL-6 is 10.39pg/ml.Patients were divided into low group and high-value group to observe each group incident which were based on the cutoff values of each marker to judge events during 6 months follow-up. The risk is more prominent in the high value of those two markers compared with low value groups(P<0.05) 2.We used logistic stepwise regression(ward) to analysis data of those patients and find out NT-proBNP ≥ 512.62pg/ml was an independent predictor of adverse outcomes(P<0.05,95%CI 1.214-28.410)3. We assessed the correlations between biomarkers and thrombolysis in myocardial infarction(TIMI) risk score by Spearman rank correlation,NT-proBNP and IL-6 are correlated with TIMI risk score(r=0.376,P<0.05)(r= 0.231,P<0.05),NT-proBNP and IL-6 are correlated as well(r=0.567,P<0.05).Conclusions:1. NT-proBNP and IL-6 could be predictors of low-moderate risk troponin-negative NSTE-ACS patients in short-term prognosis.2. NT-proBNP is an independent predictor of low-moderate risk troponin-negative NSTE-ACS patients in short-term prognosis.3. NT-proBNP and IL-6 are positively correlated relationship,and both correlated with TIMI risk score positively.
