We formulated a questionnaire for middle-aged people and evaluated comparative analyses, we tried to verify the clinical effect and value of the questionnaire of metabolic syndrome(MS), which may provide corresponding data for comprehensive intervention by finding out key points and obstacles in the behavior changes of middle-aged MS patients in China.[Methods] We formulated the survey questionnaire on MS in middle aged people by retrospectively studying related literature concerning etiology and risk factors of MS. Based on "Physical examination one hundred questions" and "Disease risk assessment questionnaire" published by Health Management Institute of the People’s Liberation Army of General Hospital, we introduced behavioral medicine into the health belief model and compiled "middle-aged metabolic syndrome general factors questionnaire" according to etiology and associated risk factors of MS.703 middle-aged subjects consisted of people receiving health examination and workers from national grid community were recruited in our study. The indexes related to MS were tested in all subjects. All subjects were divided into three groups:MS group (235), high risk group (347), and control group (235) according to the MS diagnosis criteria of China Association of Diabetes". Data analysis was performed using SPSS version 20.0(SPSS Inc., Chicago, Illinois, USA).90 subjects from each group completed survey questionnaire on MS. The data were used to evaluate the relationship between index of MS and evaluation results.[Results] ethe four dimensions and 50 catalogs were included in our questionnaire. The reliability and validity of the questionnaire were well appreciated (cronbach’a coefficient was 0.86, CVI of total score was 0.82, the CVI was 0.87-0.94 of four dimensions).②121(17.3%) out of 703 subjects were confirmed to be presented with MS.347 subjects(49.4%) were presented with one or two abnormal metabolic indexes and were categorized into high risk.239 subjects(34.0%) were presented with dyslipidemia.154 subjects(21.9%) were presented with abnormal blood glucose level. 117 subjects (16.7%)were presented with hypertension,49.3% and 14.0% middle-aged male subjects were diagnosed with overweight and obese respectively.21.0% and 5.8% middle-aged female subjects were diagnosed with overweight and obese respectively. There were significant differences between male and female subjects (x2=92.9, P<0.05).The percentages of male subjects presented with abnormal BMI, overweight/obese, high neck circumference, high waist-hip radio, and high waist ratio were significantly higher than those of female subjects. In MS group and high risk group:92%~97% subjects were presented with overweight/obese.43.6%~72.8% subjects were presented with dyslipidemia, which were much more than subjects with hypertension (9.6%) and abnormal blood glucose (19.6%).③Results of questionnaire score:The questionnaire total scores of the three groups were 28.07±5.53 (MS group), 30.39±5.5 (high risk group), and 33.23±3.67 (control group) respectively. There were significant differences of total scores, health beliefs scores, and lifestyle scores between the three groups (P<0.05). There were no significant differences of metabolic syndrome-related knowledge and health literacy score between the three groups (P>0.05). There were no significant differences of total scores, health beliefs scores, and lifestyle scores between the MS groups and high risk group (P>0.05). There were significant correlation between the questionnaire total scores and BMI, dyslipidemia, post-prandial blood glucose. No significant correlation was found between the questionnaire total scores and mean artery pressure.[Conclusions]①The reliability and validity of "middle-aged MS generafactors questionnaire" were good and is suitable to be applied into primary screening of MS and high risk subjects in middle-aged people. ②We should focus on overweight and hyperlipedimia in the middle-aged patients. Neck circumference and waist-high ratio should be listed as routine assessment for metabolic indicators, because neck circumference and waist-high ratio are more sensitive to discriminate overweight/obese when compared to waist-hip ratio and waist circumference.③the relatively low health literacy and health belief should be considered as the difficulty in changing lifestyle behaviors in middle-aged MS and high risk subjects.
