| Objective To evaluate the efficacy and safety of high frequency repetitive transcranial magnetic stimulation(r TMS) on the left side of the frontal lobe dorsolateral(DLPFC) in the treatment of depression.Methods Collected patients with depression in neurology, neurosurgery clinics in Anqing city affiliated hospital of anhui medical university from March 2013 to March 2014, collect depression are accord with the third edition of the Chinese psychiatric diagnosis and classification(CCMD- 3) diagnostic criteria for depression, CCMD- 3 is the most commonly used diagnostic criteria of a psychiatrist at present in China.96 patients with depression were randomized into three groups treated with r TMS, paroxetine combined pseudo r TMS and paroxetine. The efficacy was assessed with Hamilton Depression Rating scale-24(HAMD- 24) and the clinical efficacy of overall rating scale(CGI). As much as possible to reduce the subjective factor, Two physicians who had been trained by HAMD- 24 scale assessment inspected with the method of conversation and observation after the inspection, two physicians independent scored respectively. Two doctors score alone, respectively.All the patients test 24 Hamilton depression rating scale(HAMD- 24) after treatment 2 weeks, 4 weeks, 6 weeks, 8 weeks respectively. Treatment of patients after 8 weeks after respectively measuring scale- a clinical curative effect of judgement illness severity(CGI- SI); 8 weeks after the test of all patients clinical curative effect of overall rating scale(CGI- GI)- test effect of judgement. Clinicacurative effect according to the rate of HAMD- 24 points to determine: score 75% or higher or lower than 8 to heal, 50% or higher is effective; CGI- SI, CGI- GI secondary to curative effect. All the data( x ±s) to said, the use of SPSS 13.0 statistical software to manipulate the chi-square test and t test, P < 0.05 forthe difference was statistically significant. Results The r TMS group, paroxetine combined pseudo r TMS group and paroxetine group had 1, 2, 3 patients fall off respectively. A total of 90 patients completed 8 weeks treatment.HAMD-24,CGI- SI score of three groups were significantly decreased when compared with the baseline(P < 0.01),but there was no statistical significance between the group(P > 0.05). The effective rate of r TMS group, paroxetine combined pseudo r TMS group and paroxetine group was 78.13%81.25%,75.00% respectively, The adverse reactions was 12.5%,21.88%,21.88% respectively, all patients hadnot appeareserious adverse events. Conclusion High frequency r TMS in left DLPFC had similar therapeutic effects with paroxetine in the treatment of depressionr TMS had less adverse reaction. |
