Objective:To obsever the clinical results of using partial of the peroneus longus tendon autografts reconstructing the ankle lateral ligaments and the modified Brostr?m in treatment of the ankle lateral ligaments chronic injure.Methods : From May 2012 to May 2013,38 patients with the ankle lateral ligaments chronic injure were recruited in this study.There were 24 males and 14 females,with a mean age of 30 years(range 18 to 43years). 16 patients were treated with the modified Brostr?m procedure and 22 patients underwent the reconstruction of lateral ligaments with partial of the peroneus longus tendon autografts. Ligaments were fixed with Tunnel combined screws.The American Orthopaedic Foot and Ankle Society(AOFAS)scores 、 standard stress radiographs talartilt and anterior talartranslation were used to assess the patients preoperation and at the last follow-up.And the operation time and complications were compared between the two groups.Results: 1、operation time and general data The average operation time was 66.7±18.0min and 70.6±14.0min(P=0.752及 t=0.457,P>0.05),The average preoperative AOFAS score was 58.36±2.904 and 59.38±2.247、the talar tilt was 16.0°±1.340°and 16.23°±1.688°、the anterior talar translation was 7.44±1.153 mm and 7.45±1.01 mm.There is no serious complications and no signs of peroneal nerve injury or peroneus longus tendon rupture.2、the talar tilt and the anterior talar translation In modified Brostr?m group,the mean talar tilt angle significantly improved from16.0°±1.340°preoperatively to 6.44°±1.209°at the last follow-up, and the mean anterior talar translation significantly improved from 7.44±1.153 mm preoperatively to 5.63±1.147 mm at the last follow-up. In peroneus longus tendon group, the mean talar tilt angle significantly improved from 16.23°±1.688°preoperatively to 6.27°±1.077°at the last follow-up, and the mean anterior talar translation significantly improved from 7.45±1.01 mm preoperatively to 5.5±1.185 mm at the last follow-up(P=0.000,P<0.05).3、AOFAS score Followed-up for average 20±5.7 months and 22.6±4.3 months.The average preoperative AOFAS score was 58.36±2.904 and 59.38±2.247,The final average AOFAS score increased to 91.50±2.241 and 86.50±2.160 in two groups(P=0.000,P<0.01). The improved American Orthopaedic Foot and Ankle Society score in the peroneus longus tendon group was significantly higher than that in the modified Brostr?m group(P=0.000,P<0.01).Conclusion: 1、Using partial of the peroneus longus tendon autografts procedure or the modified Brostr?m procedure can improve the patients’ function and stablity. 2、There is no difference between using partial of the peroneus longus tendon autografts procedure and the modified Brostr?m procedure in terms of stablity. 3、The curative effect in the peroneus longus tendon group was higher than that in the modified Brostr?m group.
