The Correlation Studies Aboat CHD In Patients With Carotid Atherosclerotic Plaque Formation And TCM Syndrome Factor

Objective To study the correlation of the coronary artery disease in patients with carotid atherosclerotic plaque formation and TCM syndrome factor, for traditional Chinese medicine syndrome differentiation and treatment of the coronary heart disease to provide scientific theoretical guidance and data to support.Methods To choice 150 cases of the coronary heart disease with carotid atherosclerotic plaque formation of outpatient and inpatient, the second affiliated hospital diagnosis in shaanxi college of traditional Chinese medicine as the research object between September 2013 and March 2015, Using the method of epidemiological cross-sectional survey,applying the coronary heart disease in patients with carotid atherosclerotic plaque formation of TCM etiology and epidemiology investigation table established by group,Collecting evaluation of plaque related indicators of patients’ demographic data, the main clinical symptoms, four diagnostic information, tongue and pulse condition and carotid IMT, neck vascular stenosis degree, plaque location, nature, plaque morphology, carotid artery plaque integral,etc, to establish associated database based on coronary heart disease patients with carotid atherosclerotic plaque formation and TCM syndrome factor, to explore the related parameters of coronary heart disease patients with carotid atherosclerotic plaque formation and the correlation of TCM syndrome factor.Results In 150 cases of patients statistics from the four diagnostic data, syndrome factor classification merge situation, the relationship between carotid atherosclerosis and coronary heart disease and TCM syndrome factor relations between carotid artery atherosclerosis and four aspects.(1)Four diagnostic data: ①Distribution of TCM symptoms: chest tightness, shortness of breath, dizziness, weakness in high frequency; ②the tongue like distribution: dark purple tongue most, followed by tongue petechiae(bruises), sublingual collaterals circuity, thick greasy tongue coating, smooth the appearance of characteristics such as high frequency; ③the distribution of the pulse condition(composite pulse condition into a single pulse condition for statistics) : string is vein, sunken pulse, appear more frequency.(2)Diction syndrome factor classification mergers: qi deficiency and blood stasis, kidney deficiency, phlegm turbidity, a higher frequency.(3) the relationship between carotid atherosclerosis and coronary heart disease aspects: ①coronary artery lesion count and number distribution of carotid plaques: Coronary artery three lesions>Coronary two lesions>A diseased arteries; ②coronary artery lesion counts and IMT, plaque and stenosis degree course integral points: carotid intima-media thickness: Coronary artery three lesions>Coronary two lesions>A diseased arteries;Course integral: Coronary artery three lesions>A diseased arteries>Coronary two lesions;Narrow degree integral: Coronary two lesions >A diseased arteries >Coronary artery three lesions.(4)The relations with TCM syndrome factor and Carotid artery atherosclerosis indexes: ①Plaque nature and distribution of TCM syndrome factor: low echo plaques in patients with qi deficiency, blood stasis and phlegm turbidity, Yang see more; Strong echo plaques in patients with qi deficiency and blood stasis, kidney deficiency, phlegm turbidity, see more; Mixed echo plaques in patients with qi deficiency and qi stagnation, and phlegm turbidity, see more in poison; ②The degree of carotid stenosis and TCM syndrome factor distribution: the carotid artery in patients with mild stenosis and blood stasis, qi deficiency, kidney deficiency, and see more. Severe stenosis and blood stasis, phlegm turbid qi deficiency, kidney deficiency, see more; Severe stenosis, qi deficiency and blood stasis, kidney deficiency, phlegm turbidity, see more; ③The carotid artery plaque area and the distribution of TCM syndrome factor: plaques in carotid artery, qi deficiency, kidney deficiency, and qi stagnation and blood stasis syndrome factor; Plaques in internal carotid artery, kidney deficiency, qi deficiency, phlegm turbidity, and qi stagnation syndrome factor; Plaques in carotid artery bifurcation and blood stasis, phlegm turbidity, kidney deficiency, qi deficiency syndrome factor, etc.Conclusion The correlation of Carotid atherosclerotic plaque formation and coronary heart disease patients with TCM syndrome factor, the related ultrasonic carotid atherosclerotic plaque index provide objective scientific basis for the patients with coronary heart disease diagnosis and treatment based on syndrome differentiation in TCM.
