Foreword:Due to candida infection rates rising, the emergence of new candida pathogens, bioavailability of drugs, drug toxicity, and other important drugs incompatibility as well as development of antifungal drug resistance, scholars began to research new antifungal drugs from traditional Chinese medicines. Wubai fang,composed of bai xian pi,bai zhi,bai jiang can, bai hua teng,bai ji,ku shen,was applied to oral erosion,especially with candida infection, had certain curative effect.We planed to apply wubai fang on oral candidiasis and observe in vitro fungistasis of its effective components matrine.Objective:To evaluate the efficacy and safety of wubai fang on oral candidiasis and observe in vitro fungistasis of matrine.Methods:1. A randomized control method was designed to compared 4 groups of oral candidiasis patients(121 patients with oral candidiasis were randomly divided into 4 groups, wubai fang group(n=35),1% sodium bicarbonate gargle group (1%SB) (n=27), wubai fang + fluconazole group (n=27) and 0.9% sodium chloride (0.9%NaCl)+fluconazole group (n=32)) with improvement rate of clinical symptoms, self-conscious symptom and the negative rate of Candida albicans.2. After separation, purification and identification of 60 strains from oral candidiasis patients,MICs to matrine, fluconazole and itraconazole were measured by broth microdilution method.Results:1. Wubai fang group and wubai fang + fluconazole group were better than 0.9%NaCl+fluconazole group in effectively improving the self-conscious symptoms and the clinical symptoms, as well as 1%SB group. Wubai fang + fluconazole group of fungi negative rate was the highest (66.7%) in 4 groups. In total effective rate of the treatment, no difference between wubai fang group and wubai fang + fluconazole group otherwise the latter group was better than the other two groups. No adverse reactions was found in 4 groups.2.For Candida albicans the mic of matrine was between 390.63~3125μg/ml and average was 1562.5μg/ml.For no-Candida albicans the mic of matrine was between 195.31~1562.5μg/ml and average was 781.25μg/ml. Prompt that matrine has anti-candida effects.3.2.0% of Candida albicans in 50 strains performed resistance to fluconazole and 2.0% to itraconazole.0% of no-Candida albicans in 10 strains performed resistance to fluconazole and 20% to itraconazole.No strains to fluconazole and itraconazole cross resistance was found.Conclusion:1.Wubai fang has certain curative effect on oral candidiasis.Applying wubai fang with fluconazole can achieve ideal therapeutic effect than use it alone.2. Matrine has anti-candida effects.3. Clinical separated strains of candida have certain resistance to fluconazole and itraconazole.
