Diabetic Neurogenic Bladder Symptoms TCM Clinical Evolution Discussion

ObjectiveIn our country, diabetic increasing every year, neurogenic bladder in patients with diabetes also increase accordingly. But the research of DNB by Traditional Chinese Medicine is lack, there is no standardized treatment programs, making occur the misdiagnosis, treatment and poor conditions. This research collecting the data of diabetic with neurogenic bladder, and analysis the statistic, dialectical their type, forming a dialectical scheme for clinical reference, provides guidance for clinical treatment.Methods1 data collection and entry:the patient of endocrinology department in Guangzhou University of Traditional Chinese Medicine were Screened their residual urine volume for detailed data collection and entry of their symptoms in patients who in accordance with the diagnosis criteria.2 Analysis:The use of non-statistical partial order structure overall knowledge discovery analysis, analysis the extract of symptom commonalities between the properties of the partial ordering from the figure. Use the traditional methods of grouping patient, and then summarize symptoms characteristic of DNB patients at different stages.ResultBaseline:The total sample of 183 cases of patients who were not including 43 people,121 patients do not accordanced with the diagnosis criteria of DNB. The imaging studies of 121 patients shows that 56 have abnormal result, and theothers donot have abnormal result. There are 19patients accordanced with the diagnosis criteria of DNB, one case has impaired glucose tolerance in the final diagnosis, one case has psychological symptom, and one case whahas benign prostatic hyperplasia, which means cannot get rid of physical element. Therefore, the actual number of cases collected is 16. Searching the previous cases, there were only two cases accordanced with the diagnosis criteria of DNB.After tested the relationship between the residual urine volume and history length of diabetes, I frond that the residual urine volume and history length of diabetes has no significant correlation.The non-statistical partial order structure model diagram for finding common symptoms between syndrome of deficiency, shows that most of our cases can be generalize in the model of deficiency of Kidney Yang, the leek of Yin Ye, has not enough blood to nourish organs and muscles.ConelusionDiagnostic Features:DNB only preliminary diagnosis of "Diabetes" in first stage, in second stage, can be diagnosed as "Diabetes" and "Stranguria", the last stage should be diagnosed as "Diabetes" and "Dysuria" or "left drowning".First stage:only residual urine volume test results reach diagnostic criteria, according to Diabetes diagnosis and symptoms differentiation.Second stage:the patient has consciously urine remain or fatigue in urination of subjective or objective symptoms. Belong to Shaoyin disease, The leek of both Yin and Yang, Yin Ye loss even more, bladder cannot promoting qi circulation to induce diuresis. Or can be with Yangming heat syndrome, or ShaoYang syndrome.Last stage:with urinary retention or incontinence symptoms, according to the cases, can be divided into two symptoms:â‘  Shaoyin disease, Deficiency of Yang, retention of water-damp, Deficiency of kidney Jing.â‘¡ Yangming disease, Yangming heat, bowel obstructed, water-damp infusion into the bladder with heat, bladder cannot promoting qi circulation to induce diuresis.
