Objective:Postradiation xerostomia is common complication related to radiotherapy of head and neck tumors, and seriously affects quality of life of patients with tumors. However treatment and protective measuresats of postradiation xerostomia at Present is still at exploring stage. Therefore, ingredients like this paper to postradiation xerostomia as the research, the gargle of dark plum is applied to postradiation xerostomia in patients with head and neck tumors. The clinical effects of gargle of dark plum to postradiation xerostomia are evaluated by subjective and objective indicators about salivation of the salivary glands. Meanwhile, it could provide a valuable reference for the clinical treatment and nursing of postradiation xerostomia.Methods:60 patients in accordance with the inclusion criteria with head and neck tumors from Guangdong Provincial Hospital of Traditional Chinese Medicine between July 2013 and October 2014 were randomly delected into the observation group (n=30) and control group (n=30).The two groups were assessed by xerostomia degree accord with intensity modulated radiation therapy, and both adopt gargle method to Moist mouth, the gargle of dark plum used by the observation group and the gargle of salt water used by the control group. And these pre-meal solutions were applied 3 times daily,30minites for each time. Among 7 weeks, saliva amounts were collected and measured before and after intervention, and the RTOG radiation reaction grade standard was referenced to evaluate disease progression weekly, then basic and stimulated saliva amounts were measured by vomiting and acid stimulation methods. At the end of the course, a comparison and evaluation was made in the clinical treatment effect to postradiation xerostomia between the two groups.Results:In this study, the final valid cases were 53 the observation group (n =27) and control group (n=26)as abscission among 60 collected patients wi th 30 persons each group. Compared 7 abscission cases with all other 53 ca ses, the baseline data such as sex, age, radiotherapy planning, saliva amoun ts before intervention and RTOG scores etc, there were no statistically s ignificant differences (P>0.05)。There were slso no statistically signific ant differences between the baseline data of the observation group (n=27) and control group (n=26) (P>0.05), so the two groups is comparable. To co mpare the basic and stimulated saliva amounts of the two groups between b efore and after intervention, there were statistically significant differ ences (P<0.05).Meanwhile, Correlation analysis prompts that the amount of basis saliva before and after test in the intervention group show a posi tive correlation (P<0.05), but the regression equation has little signifi cance; whereas the amount of irritating saliva group had no significant c orrelation (P>0.05). There were statistically significant differences in multiple comparison of the basic and stimulated saliva amounts of after i ntervention (P<0.05), except comparison of the basic and stimulated saliva amounts of after intervention of the observation group (P<0.05). Signific ant statistics differences were found in the RTOG scores at the third and fouth weeks between the two groups (P<0.05),but not at the second, fifth, sixth.and seventh weeks (P>0.05). In disease type analysis, there was no statistical difference in the amount of saliva significance between the e xperimental group and the control group (P>0.05)Conclusions:1. The gargle of dark plum can effectively delay disease progress of postradiation xerostomia of head and neck tumors, espically at the third and fouth weeks.2. Compared to common western treatment such as the gargle of salt w-ater, the gargle of dark plum on the basic of guarantee clinical effect, because of its easy operation and moderate price hydropathical compress is deserved to be generalized after a series of deeper studies, and it pr-ovides a theoreticcal basis for the further development use of dark plum.3. This study could provide a new clue for the further utilization of dark plum by applying dark plum on form of gargle solution to xerostomia. |
