ObjectiveAs for the status of the current emergency infusion equipment at home and abroad, such as low safety, poor portability, strong environmental dependencies etc, this study was conducted to establish an innovative design and sample based on Quality control theory (Quality Function of Deployment, QFD) and theory of invention problem solving (Teoriya Resheniya Izobreatatelskikh Zadatch, TRIZ). This design was guided by "customer demands", and was founded on "39 general engineering parameters and 40 invention principles", integrating the individual interviews, expert meeting, analytic hierarchy process and other research methods. This sample can offer equipment for the patients with traumatic hemorrhagic shock under the special conditions, such as the traffic accident, natural disaster and local war etc.MethodsThe study can be divided into two parts.Part â… :Establishing the quality of house of the pre-hospital emergency infusion pump based on QFD.The first part can be mainly divided into two steps. The first step, based on the theory of QFD, was to integrate the methods of clinical study, individual interviews, expert meeting to collect customer demands information; then we applied situation expansion method to convert the original voice of the customers to the standardized customer requirements. The affinity diagram method was adopted to normalize hierarchical clustering customer requirements. Using analytic hierarchy process (AHP), Kano questionnaire, we determined the importance degree of customer requirements of the portable emergency intelligent infusion pump, developed the table of the customer demands deployment and provided theoretical basis for the next step, i,e, the technology features deployment. Second, based on the theory of QFD, we conducted expert consultation, group discussion, and referenced quality factors converting language expression of customers demands to language expression of designer technical characteristics. Independent collocation method was adopted to convert customer requirements importance to technical characteristic importance and we established two-dimensional table of the portable emergency intelligent infusion pump customers demand-technical characteristics, namely quality of house. The conflict between technology features was found, providing theoretical basis for the breakthrough technology bottleneck.Part â…¡:the determination of contradiction solution of the pre-hospital emergency infusion pump based on TRIZ.Based on TRIZ theory, first of all, we classified the contradiction between the technical characteristics. Second, we conducted expert consultation, group discussion and referenced TRIZ generic engineering parameters at the same time, then we converted the practical problem into TRIZ standard problem, building a standard TRIZ problem model. Third, the TRIZ separation principles and contradiction matrix was used to build the model of the conflict solution, developing the conflict solution.ResultsPart â… :the quality of house of the pre-hospital emergency infusion pump based on QFD.On the one hand, through "collecting-converting-layering-analyzing" the customer requirements of the portable and intelligent emergency infusion pump, we got the customer requirements, including 8 first level customer demands, that is safety, effectiveness, portability, intelligence, low environment dependencies etc and 36 second level customer requirements, that is bubble display alarm, infusion velocity, strong endurance capacity, heating capacity, simple operation etc. Then we determined their respective importance degree. On the other hand, through converting the customer requirements to technique features, we got a deployment table of 20 technique features, i.e. volume, weight, heat capacity, automation etc, establishing a two-dimensional table of customer requirements-technical characteristics of portable and intelligent infusion pump, namely quality of house, and found 5 technique feature conflicts of flow rate, screen size-volume, maximum velocity-volume, maximum velocity-weight, maximum velocity-heating capacity.Part â…¡:contradiction solution of the pre-hospital emergency infusion pump based on TRIZ.Through constructing TRIZ standard problem model, we converted 5 general technique characteristic conflicts (flow rate, screen size-volume, maximum velocity-volume, maximum velocity-weight, maximum velocity-heating capacity.) to 5 TRIZ standard problems (speed, area of stationary object-volume of stationary object, speed-volume of stationary object, speed-weight of stationary object).Using No.19 TRIZ principle of the invention i.e. periodic action to solve the core technology bottleneck (the technical issues between flow rate and volume/weight), we developed a sample of portable and intelligent emergency infusion pump.ConclusionBased on QFD and TRIZ theory, this research formed a deployment table of customer requirement of the portable and intelligent emergency infusion pump including 8 first level customer demands(i.e. safety, effective, portable, intelligent, low environment dependencies etc) and 36 second level customer requirements(i.e. bubble display alarm, infusion velocity, strong endurance capacity, heating capacity, simple operation etc). A deployment table of technique features was formed, including 20 technique features(i.e. volume, weight, heat capacity, automation etc).5 general technique characteristic conflicts (flow rate, screen size-volume, maximum velocity-volume, maximum velocity-weight, maximum velocity-heating capacity.) and 5 TRIZ standard problems (speed, area of stationary object-volume of stationary object, speed-volume of stationary object, speed-weight of stationary object) was found. Using No.19 TRIZ principle of the invention(i.e. periodic action), we solved the core technology bottleneck. The results of this study can provide theoretical basis for the research of emergency infusion pump under special condition such as accident, war, disaster and so on, help to develop a portable and intelligent emergency infusion pump guided by "customer demand", "39 general engineering parameters and 40 invention principles".
