| Objective Inductive nearly 2 years of undergraduate course room I implementation of 45 cases of application free forearm flap repair cases of oral malignant tumor tissue defect, on the use of the intraoperative compares two different methods of vascular anastomosis.Methods Use forearm free flap surgery earlier repair tissue defects of oral cancer,25 cases using the traditional method of hand-stitching magnifying anastomosis,20 cases using microvascular anastomosis device (GEM COUPLER) agreement, respectively named manual and stapling group, comparing flap area, vein anastomosis time, postoperative complications and flap necrosis rate differences in the two groups.Results Free flaps in 45 cases,10 cases of postoperative complications, 22%of the total operation,9 cases are come from manual group,1 case come from stapling group. The complications of manual group include:1 case of postoperative blood circulation disorders, skin flap necrosis; 5 cases of venous crisis, after rescuing the flap survival; 3 cases of postoperative infection, flap survival after inflammation control. The complications of stapling group isvenous crisis, after rescuing the flap survival. Overall flap survival rate of 97.8%. Use statistical methods to determine the two groups flap area, the difference vein anastomosis time, postoperative complications and flap necrosis rate between whether it makes sense, venous anastomosis time difference between the incidence of postoperative complications and the there was statistically significant (P<0.05), while the difference between the flap area and flap survival was not statistically significant (P> 0.05).Conclusions Microvascular anastomosis device (GEM COUPLER) can reduce vein anastomosis time, simplify the surgical procedure, thereby reducing the flap ischemia time, reduce the incidence of postoperative complications, is convenient and reliable venous anastomosis. |
