Objective: To detect dynamic changes of IL-27 in serums of chronical viral hepatitis B, liver cirrhosis and acute-on-chronic liver failure associated with hepatitis B, and analyze the relation of Hepatic function indexes, immunological functions and IL-27 levels in different stages of disease, in order to explore the effect of IL-27 on chronical viral hepatitis B and provide scientific value for detecting disease and guiding medication in clinical viral hepatitis B patients.Methods:1 There were 89 cases of clinical viral hepatitis B(CHB), patients(31 cases with light patients, 51 cases with moderate patients and 7 cases with severe patients). In additon, 41 cases were included in liver cirrhosis(LC,12 cases in the compensatory stage, and 29 cases in the decompensatory stage) patients.There were 36 cases in the stage of acute-on-chronic liver failure(ACLF). The 10 cases healthy people in the examination were involved as control group.2 We drew fasting venous blood, added 5ml venous blood into a tube, and then collected the serum specimen an storaged it in frige at-80℃.Other blood were used for liver function tests and immunological functions, such as ALT, TBIL, ALB and so on. The HBV-DNA load were detected by real-time fluorescent quantitation polymerase chain reaction, according to description of the kit. Besides, prothrombin time activity were detected from five milliliter of peripheral blood in a specified tube.3 According to assay procedure in the introduction of enzyme-linked immunosorbent assay kit, We dispensed 100μl of standards and samples into the appropriation microtitre wells, read the optical density at 492 nm with a microtiter plate reader, constructed a standard curve by plotting the mean absorbance obtained for each reference standard against its concentration in pg/ml on linear graph paper. The serum levels of IL-27 and IFN-γ in the samples were detected.4 The liver tissues were biopsied in the 57 cases of chronical viral hepatitis B patients trough paracentesis technique according to clinical routine procedure.5 The relationships of IL-27, IFN-γ and Hepatic function indexes were analyzed in the serums of different HBV infected patients. In addition, the levels of ALT, HBV-DNA, IL-27 and clinical values were evaluated in the serums of patients.Results:1 The comparison of IL-27 and IFN-γ in the serums of different HBV infected patientsThe serum levels of IL-27 and IFN-γ were higher in different HBV infected patients(P<0.01).2 The analysis of the relationship about the levels of IL-27 and liver function tests, IFN-γ in different HBV infected patientsThe relationship among IL-27 and liver function tests or IFN-γ in different HBV infected patients were analyzed by rectilinear correlation.The results showed that there were positive correlation between IL-27 and IFN-γ,ALT,TBIL(P<0.05). However, there were negative correlation between IL-27 and PTA,ALB(P<0.05).3 The relationship between inflammatory classification in the hepatic tissues and IL-27 in the serumsThe levels of IL-27 were not dependent with inflammatory classification in hepatic tissues. There were no obvious difference statistically among groups(P>0.05).4 The relationship between fibrosis in the hepatic tissues and IL-27 in the serumsThe hepatic tissues of 56 cases occurred different levels of fibrosis after received liver biopsy technique. There were no correlation between the levels of fibrosis in the hepatic tissues and IL-27 in the serums of patients(P>0.05).5 The relationships between prognosis of acute-on-chronic liver failure and IL-27 in the serums of patients11 cases in the 36 cases of acute-on-chronic liver failure patients were survival, and 25 cases were dead. The levels of IL-27 in the serums from the former were lower than the latter, but the difference was not statistically significant(P>0.05).6 The relationships between MELD Scores and IL-27 in the serums of patientsThe patients of liver cirrhosis with compensatory or decompensatory stage and acute-on-chronic liver failure were scored by MELD standard. The results showed that the levels of IL-27 in the serums from the exceed 40 group were higher than from the less than 40 group. The difference was statistically significant(P<0.05)7 The relationships between HBe Ag and IL-27 in the serums of patientsThere were 90 cases of HBe Ag positive patients and 86 cases of HBe Ag negative patients. The difference of the levels of IL-27 was not statistically significant(P>0.05)between the two groups.8 The relationships between the levels of duplicate HBV-DNA and IL-27 in the serums of patientsThe patients were divided into three groups according to different levels of HBV-DNA, including 60 cases of less than 5log10 copies per ml, 47 cases of more than 7log 10 copies per ml, and 69 cases of intercalary group. There was no statistical significance about the levels of IL-27 among the three groups(P>0.05).Conclusions:1 IL-27 may increase the level of IFN-γ as Th type I cytokine, and enhancing the effection of clearing HBV viral by promoting cellullar immunologic response. It also may be one of the mechanism for damage of hepatocytes in patients with HBV infection and refects serious degree of hepatocytes damage at a certain extent, but no effection on the levels of HBV-DNA.2 IL-27 may be related to deterioration of chronical viral hepatitis B.It can be used as a predication index for prognosis of chronical viral hepatitis B.
