| Objective: Vertebroplasty has been applicated on the clinic so long time,its effect is received among the clinical physicians and patients.The scope of vertebroplasty application get further promotion by the time,also the clinical complications was highlighted in the clinic and were reported by the scholars at home and abroad,however,there is few large clinical data analysis report In the domestic.Collect our hospital of patients with vertebroplasty relevant clinical data were retrospectively analyzed,with data show that the operation of clinical application, to clarify its advantages and disadvantages,in order to make the operation to better serve the clinical patients and this kind of operation was acquired by the clinical surgeons.Methods: By the electronic medical records query system, retrospectively studying the clinical data of 1084 patients with percutaneous vertebroplasty from January 2003 to March 2012, among the 1084 patients, only 1080 cases meet the demand with data integrity. Among the cases,the author detailedly referred to the hospital volunteers, course records, operation records, anesthesia record sheet, auxiliary examination report and preoperative postoperative imaging data.Paying attention to 12 basic elements respectively that includes age, gender, diseases,the number and the distribution of surgical vertebral body,the X-ray of postoperative patients, the injection quantity of vertebral body bone cement, the leakage rate of bone cement in the spinal canal and out of the vertebral body, the number of patients who were performed vertebroplasty or kyphoplasty, the amount of patients with vertebroplasty of unipedicular or bipedicular, the vital signs of patients during the operation and so on. In the meantime, focusing on the analysis of the patient’s clinical manifestation and the living quality after the surgery.Results: Among the 1080 cases, there are 964 cases of Osteoporotic compression fractures and 81 cases of old fracture, 6 cases of malignant tumors,4 cases of spinal canal placeholder, multiple myeloma in 2 cases, 21 cases of pathological fracture, langerhans histiocytosis in 1 case, non-hodgkin’s lymphoma in 1 case; Single vertebral body in 638 cases, double vertebral bodies in 221 cases Three vertebral bodies in 123 cases, more than three vertebral bodies in 98 caes; Census number of every vertebral body including 0 case in C1,0 case in C2,2 cases in C3,2 cases in C4,3 cases in C5,2 cases in C6,3 cases in C7,3 cases in T1,0 case in T2, 2 cases in T3,3 cases in T4,9 cases in T5,18 cases in T6,48 cases in T7,61 cases in T8,64 cases in T9,64 cases in T10,184 cases in T11,403 cases in T12,441 cases in L1,246 cases in L2,142 cases in L3,108 cases in L4,32 cases in L5,1828 cases in total;there are 306 patients that have the symptom of bone cement leakage through the X-ray after the surgery and it accounts for 28.33%; the injection quantity of bone cement varies from 1.5ml to 7.5ml; it is zero to find the leakage in the spinal canal during surgery; there are kyphoplasty 706 patients and it accounts for 65.37% of the total vertebroplasty; there are 984 cases who have unipedicular vertebroplasty and it accounts for 91.11% of the total vertebroplasty; 23 patients appeared clinical symptoms with 3-5 days after the surgery, 3 cases had neurologic symptoms,the other 20 cases shows the clinical signs of pulmonary embolism including 5 males and 15 females, among these 20 cases, there is one patient that had transferred to the respiratory medicine department in our hospital because of the severe pulmonary embolism symptom; in these 23 patients, 8 cases had the unipedicular vertebral body surgery and 15 cases had the bipedicular vertebral body surgery; 17 patient had the thoracic surgery.Conclusion: Old people who complain thoracic and lumbar discomfort, should be highly suspected vertebral compression fractures. After all inspection and pre-operation evaluations,the patient who are in accordance with operation indications could acqire satisfied clinical curative effect by the standard operation.Vertebroplasty although there are a variety of clinical complications,but the surgical method and strictly operation indication,the operation should be carried out in clinical positively. |
