| China is one of the Tuberculosis high-burden countries. In order to respondeffectively to the new challenges of Tuberculosis control, the Tuberculosis preventionand control mode in China is now in the process of transformation. Under the new mode,Tuberculosis designated hospital becomes the main agency responsible for the diagnosisand treatment of Tuberculosis. This transformation has great influences on theTuberculosis physicians in the Tuberculosis designated hospitals, since their mainresponsibilities are changed greatly. This study aims to discuss the capacities which theTuberculosis physicians should have under the new Tuberculosis prevention and controlmode, their current level of competency and its influencing factors, so as to providereferences to the human resource development of the Tuberculosis physicians.Through questionnaire survey and depth interview on the Tuberculosis physiciansin Zhenjiang, Yichang and Hanzhong, which are the three pilot areas of the newTuberculosis prevention and control mode, this study integrates the principles ofCompetency Model and survey results to build a Competency Model for Tuberculosisphysicians, which includes solid mastery of knowledge, knowledge application, masteryof certain knowledge, specific knowledge, identify disease, prevent complication andidentify other infectious diseases, mastery of the comprehensive treatment ofTuberculosis and AIDS, knowledge and ability of rational drug use, self development,grasp the real-time information, mastery of the knowledge concerning regionalinfectious disease, familiar with the conditions of patients, the physician-patientrelationship, communication skills, general health education and behavioral intervention,protection of special populations (women and children), the health education andbehavioral intervention of the special patients who have other infectious complications,facing pressure ability, professional ethics, and professional commitment.Based on the Competency Model for Tuberculosis physicians, this study carriedout a survey on the Tuberculosis physicians in Zhenjiang, Yichang and Hanzhong.Based on the sample data, firstly, this study does descriptive statistical analysis on thecompetency of the Tuberculosis physicians. The study found that in both thecounty-level general hospitals and the municipal-level specialist hospitals in the pilotareas, the competency of the Tuberculosis physicians is about the perceived eligibility level, and for some key elements, their competency needs to be improved. Secondly,this study also tried to explore the influence of educational background and workingexperience on the competency of the Tuberculosis physicians, and found that for theTuberculosis physicians in the county-level general hospitals and the municipal-levelspecialist hospitals, clinical working experience in Tuberculosis prevention maybe animportant factor which will affect the competency of the Tuberculosis physicians. Andeducational background maybe not an important influencing factor.From the research outcome, this study puts forward some policy suggestions on thecompetency advancement of the Tuberculosis physicians under the new Tuberculosisprevention and control mode. First, complete the salary system and welfare system ofthe Tuberculosis physicians, and improve job attraction. Second, carry out standardtraining for Tuberculosis physicians, and improve the capacity and quality of theexisting Tuberculosis physicians. |
