| ObjectiveThis study aims to reconstruct the community nurse position training curriculuminclude course, content and class hour by means of analyze and evaluate thecommunity nurses position training programs in a community nurse position trainingcenter in Guangzhou. Standardized the community nurses position training by thereconstruct curriculum, meanwhile, provide a more complete teaching guidelines.Method1. On the basis of the literature review self-design the questionnaire, analyzereliability and validity of the questionnaire, and investigated a community nursetraining center of Guangzhou’s existed curriculum design by the questionnaire.Questionnaire have Student version and Teacher version. Using cluster sampling andfield survey method investigate the413nurses who came from Panyu District ofGuangzhou and have participated in the community position training as well as the20training teachers during the2009~2012. Input the questionnaire data by Epidata3.1,and using SPSS17.0statistical software analyze it, statistical methods includedescriptive statistics, rank sum test, spearman correlation coefficient.2. Reconstruct the community nurse position training theoretical curriculum ofGuangzhou include course program, course content and class hour by Delphi method.Set up the task team, prepared the expert consultation questionnaire was based on theexisted training curriculum and the result of the prevalence survey. Giving thequestionnaire to26experts whose major is related fields and meet up with theselection criteria by Email, and conduct3rounds consultation. Using Excel2007andSPSS17.0software for data adjustment and analysis. The general condition of expertswas represented as percentage, the experts’s positive coefficient was represented asthe effective return rate and propose suggestion rate, the experts’ authority level wasrepresented as authority coefficient(Cr), the experts’s level of consensus wasrepresented as importance mean, the experts’ level of coordination was represented as coordination coefficient(Kendall’s W). Moreover, based on the result of consultationreconstruct the community nurse position training theoretical curriculum ofGuangzhou combined with the actual condition.Result1. Index of content validity of prevalence survey questionnaire both Studentversion and Teacher version are0.979, test-retest reliability of Student version is0.973.2. The Student version questionnaire was sent361, and returned353, theeffective rate is88.9%; the Teacher version questionnaire was sent20, he effectiverate is100%.3. The Student version nurses age94.8%concentrated in20~29, the average ofclinic nursing seniority was5.28±5.02year, the average of community nursingseniority was1.88±3.34year,43%of nurses had community nursing experience.4. The surveyed teachers the average of seniority was≥15years,12(60%) ofthem participated community nursing teacher training. Within the20teachers,engaged in community nursing are3(15%), engaging in community nursing are4(20%), never engaged community nursing are13(65%).5. The average of evaluation score of each course importance and practicabilityare>4(Total is5) by nurses. Nurses for the curriculum overall satisfaction were88.4%. Teachers for the curriculum management overall satisfaction were65%.6. The evaluation of course importance had no difference at education, workingmode, hospital category, current position of nurses, the difference was not statisticallysignificance, P>0.05.7. In9courses, hospital category to impact evaluation of course practicability ofnurses, the difference was statistically significant: General medicineintroduction(2=8.470,P=0.037), Community health service introduction(2=10.548,P=0.014), Vital signs monitoring(2=8.012, P=0.046), PICC FAQ andTreatment(2=15.943, P=0.001), Community emergency and first aidtechniques(2=8.972, P=0.030), Behavior and health of Community population(2=10.038,P=0.018), Hospice care(2=12.551,P=0.006), Hot and Coldtherapy(2=22.900,P=0.000), Home peritoneal dialysis(2=15.851,P=0.001).8. In3courses, clinic nursing seniority to impact evaluation of coursepracticability of nurses, the difference was statistically significant: Communitypopulation and nursing intervention(rs=ï¼0.125,P=0.026)ã€Diabetes communitynursing practic(ers=ï¼0.116,P=0.038)ã€Hot and Cold therapy(rs=ï¼0.118,P=0.036).9. In12courses, community nursing seniority to impact evaluation of courseimportance and practicability of nurses, the difference was statistically significant:General medicine introduction, Community health service introduction, Breastself-examination, Vital signs monitoring, Skin and Enterostomal nursing, PICC FAQand Treatment, Diabetes community nursing practice, Community emergency andfirst aid techniques, Infectious disease prevention\nursing intervention andoccupational protection, Hospice care, Hot and Cold therapy, Community patientmedication, Home peritoneal dialysis(Table2-8).10. It have22experts participated the Delphi consultation, came from grade-3pandocheum(9,40.9%), nursing university(6,27.2%), community health center andprimary hospital(7,31.8%) in Guangzhou. The average of age was44.82±5.65yearsold, the average of seniority was24.36±6.53years. Within the22experts, there were21experts have vice senior and senior title,7Doctoral degree,7Master degree,6Bachelor degree,2Faculty degree. Background were nursing(18), public health(2),clinical(1), general medicine(1).14experts were “very familiarity†and “familiarityâ€for community health service condition in Guangzhou.11. The effective return rate for3rounds were84.6%,86.3%,78.9%, the averageof effective return rate were83.3%, the propose suggestion rate were55%,53%,27%.Three rounds consultation the average of judgment evidence coefficient(Ca) were0.834ã€0.844ã€0.845, the average of familiarity(Cs) were0.754ã€0.749ã€0.725,Coefficient of Variation(Vj) were0.20ã€0.19ã€0.17, authority coefficient(Cr) were0.793ã€0.796ã€0.787, Kendall’s W were0.195ã€0.289ã€0.407. 12. After consultation adjusted11courses, deleted3courses, added9courses,formative curriculum have26compulsory courses and2optional courses,100classhours.Conclusion1. Nurses had high evaluation of overall acknowledge and satisfaction for thecommunity nurse training center curriculum design, but the curriculum structure needmore improve.2. Nurses’ workplace whether in community and the nurses whether havecommunity nursing experience were important effect factors for curriculumevaluation, it indicate the community nurses and clinic nurses have different opinionat the same problems, the need of community nurses should be primary in training.3. The theoretical courses content were the focus of the training curriculumdesign, it indicate the curriculum design need more attention to the content ofpracticality in the position training, the training content should be based on the needof community nurses, and the practicability of community-oriented, show differenceof community nursing and clinic nursing.4. It is essential measures that we should enhance teachers’ level, rich trainingmethods and strengthen practice for improve the community nurse position training.5. In the study, the selected experts had strong representativeness, high positivity,good authority and compatibility, reconstruction of the <Community nurse positiontraining curriculum of Guangzhou> had reliable. It may as a guidance for communitynurse theoretical training in Guangzhou. |
