| Background With the enhancement of people’s living standard and the demand for better health care, a lot of opportunities have emerged for the development of the departments specialized for the health care of women and children. The situation for woman and child health care in Nantong City has been improved significantly since under the guidance of the government policy to care most people health as the center and protect the reproductive health as the aim with the combination of health care and clinic. However, currently there are still a lot of problems for the survival and development of the departments engaged in women’s and children’s health. In this study, we investigated the county Departments of Women’s and Children’s Health, performed the analysis on their operation, identified some problems and put forward some proposals.Objective To find out the problems in the operation of the county health care institutions of Women’s and Children’s Health in Nantong City and provide some appropriate measures to solve the problems.Methods1. Quantitative study:The retrospective data were collected from5county health care institutions of Women’s and children’s Health in Nantong City in2005,2007and2010.2. Qualitative study:Three groups of people were investigated with different purposes:For evaluating the annual performance of the5hospitals, the senior administrative who are currently in charge of all the county hospitals in Nantong City were first surveyed. The local administrative, doctors and nurses in the5hospitals were then surveyed to know the basic situation for running. Finally, to know the capacity of the hospitals to provide health care and solve the related problems, we carried out the survey on some midwifes working in some small hospitals affiliated to the hospitals of women’s and children’s Health. All the surveyed participants are willing to participate for this study and express their own views.Results 1. The absolute government financial input to the hospitals of Women’s and Children’s Health was increased annually. However, the proportion of the input to the total government financial investment was decreased.2. There was no significant difference among the annual survey on the staff number, real estate value and area as well as the proportions of the non-administrative and high-educated and the age structures.3. With more government financial input, the fare charged in every hospital together with the proportion of income and expenditure was decreased year by year.4. Region D had the largest real estate area and staff number with the highest per capita income among the5hospitals. Region B had the highest real estate value but together with significantly decreased government input and increased proportion of income and expenditure.5. Every hospital worked well for prenatal examination and systematic administration for the pregnant and puerperal women and infants below3-year old. They also did well in postpartum and postnatal follow-up respectively for women and infants and surveys on woman’s disease. For the mortality of pregnant and puerperal women together with infants and child below5-year old, everything is controlled in a reasonable range. However, the premarital medical examination and3-7-year old children’s vision screening had significant difference among the5regions due to the different attention from government and different operation period.6. After comprehensive evaluation from human resources, material resources, financial resource and the income and expenditure of the investment for the hospitals, Region D was the best and Region C was relatively the worst.Suggestion1. It is essential to emphasize the responsibility of every hospital for woman and child health care and optimize the construction of the team to provide better health care to women and children. It is also necessary to increase more financial investment and establish a new standard for the evaluation of staff performance.2. The capacity of every department for providing better health care to women and children should be strengthened with the direction towards services for most people, major public health projects, two planned aims and fair opportunities for public health care. For the Hospital of Women’s and Children’s Health itself to provide better health care service to women and children, it should be reestablished an new evaluation standard with more incentives to promote development and recruit more talents with more training to make the team strong.3. The ability and the level for the services to women’s and children’s health should also be enhanced by more education on health and self-care awareness. The field for health care to women and children should be gradually expanded by research, integration, translation, promotion and application to solve the outstanding and urgent problems in the work. |
