Objective To determine the best effective range of the volumetric ratio of lipiodol-ethanol mixture (LEM) by comparing the therapeutic efficacy of the different volumetric ratio of LEM on rabbit VX2-hepatic carcinoma treatment.Materials and Methods Firstly. the New Zealand white rabbit VX2-hepatic tumor model was established. After the tumor-bearing rabbit was generally anesthetized, the tumor mass was peeled and then cut into1-2mm3small pieces under aseptic conditions. which was implanted in the left hepatic lobe under CT-guided. Two weeks later, the successful tumor models were divided into six groups after GE1.5TMR examination. Secondly, puncture the right femoral artery under direct vision, and place the micro-catheter into the aorta. Angiography of celiac artery and hepatic artery was performed, then super selectively place the micro-catheter into the left hepatic artery and infuse the lipiodol-ethanol mixture. Postoperatively, ligation of the right femoral artery was performed, and antibiotic was given to prevent infection for three consecutive days. Then tumor models were executed after one week. and the specimens were fixed with neutral formalin, embedded in paraffin. and HE staining and immunohistochemical staining were dyed, respectively. After screening,18cases were chosen for this experiment. Tumor models were divided into A-F six groups,3cases in each group. And from group A to group F, the volume ratios were3:1,2:1,1:1,1:2,1:3,1:4,.Results On MR diffusion-weighted images, the signal of postoperative lesions became lower than the preoperative lesions. In groups B~D, the postoperative volume values were reduced, and changes of their preoperative and postoperative volume values have statistical difference (P=0.008<0.05). TGR ranges from-52%to-97%. In group A, E and F, the postoperative volume values were increased, and changes of their preoperative and postoperative volume values have statistical difference (P=0.008<0.05), TGR ranges from32%to176%. HE histopathology indicated that there was much residual carcinoma tissue in group A, and coagulation necrosis occurred only in the peripheral portion of the tumor nest, but cancer cells grew and divided vigorously in the central portion of the tumor nest. In group B-D, coagulation necrosis could be seen in the most of the tumor tissue, and the difference is not obvious in the size and shape of the residual cancer cells. In group E and group F. hepatic cords could be seen in the most area of the coagulation necrosis, but tumor necrosis occurred seldom, and invasion of the normal hepatic tissue could be seen. MVD values have statistical differences among groups A-F, P=0.01<0.05.Conclusion This experiment shows that the higher proportion of ethanol, the stronger on tumor angiogenesis inhibition; and when the volume ratios of LEM range from1:2to2:1, the therapeutic effect is better. Based on these results, the therapeutic efficacy of the1:2ratio group is best.
