ObjectiveTo investigate the prevalence and clinic attendance of patients withcognitive dysfunction in neurological outpatient department in the regionand analyze the reason underlying low outpatient rate, so as to recognizepatients with mild cognitive impairment as early as possible and improvelow outpatient rate of patients with cognitive dysfunction.MethodsThe cognitive function of outpatients conforming to the selected criteriain the neurological outpatient department of the second affiliated hospital ofChongqing Medical University from Apr.4,2014to Apr.15,2014wasinvestigated on the spot. Informant Questionnaire on Cognitive Decline inthe Elderly (IQCODE) and Montreal cognitive assessment (MoCA) wereused to screen. If score of IQCODE is greater than or equal to3.3; scale ofMoCA was further to be processed. If the score of MoCA is less than26,the patient was recognized as one with cognitive impairment and proceeded to complete the survey of questionnaire on the spot, Collected data wasused to compare and analyze the cause of failure to see a doctor timely bySAS8.1statistical software with single factor method.ResultsThere were altogether1,042outpatients attending the neurologicaloutpatient department in the survey stage.409patients conforming to theincluding criteria completed IQCODE.115patients with scores ofIQCODE greater than or equal to3.3completed MoCA,55patients wererecognized as ones with cognitive impairment, of whom15ones went tosee a doctor with symptoms of cognitive impairment (CI) and40oneswith other symptoms. The prevalence of patients recognized as ones withCI among patients at and over the age of55years was about5.28ï¼…andthat among patients attending the neurological outpatient department was1.44%. There was significant difference in marital status, the educationallevel, awareness of dementia, economic condition and inconvenience ofmedical facilities among the group visiting with CI symptoms and onevisiting with non-CI symptoms. Meanwhile, there was no significantdifference in age, gender and occupation. Concerning awareness ofdementia knowledge, there is a significant difference between the twogroups in early performance and causes of dementia, while no significantdifference in preventive and treatment effectiveness of mental activity. Inthe group ones visiting with non-CI symptoms,the accumulative total of the four potential causes leading to low outpatient rate: lack of dementiaknowledge (49.38%), economic difficulties (67.50%), inconvenience(50%),other reasons (10%).There was no significant difference on thedegree of cooperation of propaganda to dementia, patient sources andmedical knowledge sources between the two groups.ConclusionsOur hospital has a high recognition rate and low visiting rate of CI inneurological outpatient department. The low outpatient visiting rate of CIwas related with marital status, degree of education, ignorance of dementia,economic reason and inconvenience. The visiting rate of married patientswas significantly higher than that of the widowed or divorced ones and thevisiting rate of patients with high educational level was significantly higherthan that of low educational level. Ignorance of dementia was mainly in theaspects of early manifestations and causes of cognitive impairment. Finally,the patients with CI in outpatient department have a low cooperation degreeand propaganda of common knowledge of dementia needs to bestrengthened.
