Discussion HE4, CA125and ROMA Diagnostic Value Of Serum Index In Epithelial Ovarian Cancer

Objective:To investigate the value of serum human epididymis protein4(HE4), the carbohydrate antigen CA125and ROMA index in the diagnoses of ovarian cancer among women. Methods:184cases of epithelial ovarian cancer HE4and CA125serum samples were tested by using chemiluminescent, which resulted in ROMA index of menopausal status;132cases of benign ovarian disease and107healthy subjects as control groups. Results:1.HE4detectable levels in premenopausal and postmenopausal patients with malignant group were higher than the healthy control group and benign ovarian disease group, but no significant difference in the control group with benign disease are found. CA125levels was detected in the three groups, the differences were statistically significant (P<0.05).2.HE4diagnostic specificity and positive predictive value is higher than CA125; their joint detection diagnostic sensitivity and diagnostic specificity are92.39%and81.06%,87.66%and the accuracy is high in each individual test.3HE4positive pathologic staging in stage Ⅰ+Ⅱ period is higher than CA125,CA125positive rate in the period of Ⅲ+Ⅳ in HE4, joint detection positive rate in Phase Ⅰ+Ⅱ period is92.68%;4. There are no significant differences of the positive rate between the Uighur people and the Han people while in the separate test or joint test of the HE4and CA125before and after the menopause; except there is no statistical significance of the positive tate of CA125while between age≤45years and>45years of the target people, positive rate are statistically significant.5.1n the HE4and CA125joint detection Fisher discriminant analysis with the correct identification of ovarian cancer in premenopausal group rates were78.82%(67/85), postmenopausal group79.80%(79/99).6. In the premenopausal group, ROMA index of diagnostic sensitivity and specificity are90.59%,89.06%respectively; postmenopausal group, ROMA index of sensitivity and specificity are91.52%,86.76%respectively, the difference between the two groups are not statistically significant Χ2=0.26, P=0.873.7.CA125, area under the curve HE4, ROMA index HE4and ROMA are the area under the curve is greater than CA125in premenopausal and postmenopausal groups.Conclusion:1.CA125distinguish between benign and malignant ovarian disease is not as good HE4, HE4ovarian cancer can be applied to reduce the rate of misdiagnosis improve early diagnostic capabilities, its diagnostic value of combined detection of ovarian cancer higher.2.ROMA index suitable for assessing the risk of epithelial ovarian cancer, thereby improving diagnosis.
