ObjectiveBy multislice CT MPR and VR reconstruction technique to observe chronicmiddle ear mastoiditis patients normal controlled morphology and pathologicalchanges of ear auditory ossicle chain ear bone damage,and the imagingdiagnosis and surgical results of contrast,compare the twoconsistency,Investigate the value of multi-slice spiral computed tomography(MSCT) three dimension reconstruction in the diagnosis of middle ear andmastoid fine structure’s bone destruction in patients with chronic otimastoiditis.Method68patients with chronic otimastoiditis were performed temporal bone thinslice scan using128layer spiral CT scanner, transparent VR,MPR.Thesepatienis were from inpatients of the department of otolaryngology of the firstaffiliated hospital of Liaoning Medical;then observed the normal morphologicalstructure of ear ossicles in normal control group,and compared the structure andbone destruction of middle ear and mastoid with surgical findings in diseasegroup,as well as the bone destruction of the auditory ossicle chain.ResultsNormal VR,MPR bones of the hammer,hammer shank bone,incus long legs,incus short legs and incus body can clearly show.There are differences between the display of the hammer and anvil joints,P<0.05.The difference hasstatistically meaning.MPR is slightly better than the VR.MPR can partly displaystirrup anvil joint,kicking bones forefoot,kicking bones backfoot and kickingbones floor.VR can not display stirrup anvil joint,kicking bones forefoot,kickingbones backfoot and kicking bones floor,P<0.05.The difference has statisticalsignificance,indicating that MPR has obvious advantages.Compared with surgical findings,Kappa values of lesions hammer,anvil,hammer anvil joint entrance drum sinus,sigmoid sinus bone wall and mastoidbone destruction CT diagnosis,are bigger than0.75,indicating that both of themhave very high consistencies.Kappa values of stirrup and stirrup anvil joint bonedestruction are0.565and0.559.Consistent of the degree is acceptable.ConclusionMSCT three dimension reconstruction can clearly show the fine structure ofmiddle ear,and diagnosis otimastoiditis bone destruction accurately,but showstapes,stirrup anvil joints and their bone destruction poorly. |
