Clinical Curative Effect Observation On Treatment Of Diabetic Peripheral Neuropathy With Principle Of Invigorating Yang And Restoring Collateral

Purpose:Through randomized controlled clinical trials,Observe the effects of clinical symptoms and signs of principle of invigorating yang and restoring collateral on diabetic peripheral neuropathy (DPN) patients before and after treatment.Monitor the changes of blood glucose,glycosylated hemoglobin (HbAlc),nerve conduction velocity (NCV),nitric oxide (NO), tumor necrosis fator-a (TNF-α).To explore the clinical significance of principle of ingorating yang and restoring collateral with DPN control.Methods:Using random Parallel comparison research method,64cases of patients with2diabetes peripheral neuropathy who pertain to the syndrome yang asthenia of spleen and kidney, phlegm and blood stasis in the collaterals, were randomly divided into two groups. Of33patients were taken into the treatment group and31patients were taken into the control group. On the basis of lifestyle intervention and western medicine,the control group was given in Mecobalamin Tablets and Epalrestat, while the treatment group was treated with herbal decoction by principle of invigorating yang and restoring collateral and Mecobalamin Tablets, Epalrestat.4weeks for a course of treatment. After3courses, observe the symptoms and signs of patients with DPN before and after treatment, at the same time examinate fasting blood glucose (FBG), postprandial plasma glucose (PBG), HbAlc, NCV, NO, TNF-a.Results:(l)The improvement of symptoms and signs:the treatment group patients after treatment, the symptoms were obviously improved, with the total effective rate of86.7%, the control group total effective effectiveness for60%, TCM symptoms improved conditions in treatment group is better than in control group (p<0.05);(2)Hypoglycemic effect:Treatment group and control group patients, FBG,PBG and HbAlc are than before treatment significantly decreased(p<0.01).The treatment group patient FBG,PBG and HbAlc is group compared with difference(p<0.05);(3) Electromyography:treatment group patients with right median nerve, motor never, sensory nerve conduction velocity are relatively before treatment improved significantly(p<0.01), right tibial nerve, motor never, sensory nerve conduction velocity are relatively before treatment improved significantly (p<0.01), Comparison between groups, the treatment group patients with right median nerve, tibial nerve, motor nerve sensory nerve conduction velocity,amplitude improvement are superior to that of the control group (p<0.05);(4) Serum levels of nitric oxide and tumor necrosis factor-a:two groups of patients after treatment, Serum nitric oxide indexes than before treatment improved significantly (p<0.01), Contrast between both groups, the curative effect of treatment group was significantly better than control group(p<0.01). Both groups have not seen the adverse reactions. Conclusion:Using principle of invigorating yang and restoring collateralin the treatment in type2diabetes peripheral neuropathy, can not only obviously decrease the TNF-a level of p,still can increase the NO in the blood and enhance the nerve conduction velocity,delay the progress of DPN.
