Application Of Continuity Nursing In Patients With Leg Venous Ulcer

Objectives To study the feasibility and the intervention effect of the continuity nursing in patients with venous leg ulcer。Methods Patients were divided randomly into two groups (32cases in the experimental group and31cases in the control group), in period from June2012to December2013. Those patients with lower limb vein ulcer came to center for changing fresh dressing on the face of wound and switched back to the local nursing treatment. The control group received regular phone as a return visit, and the intervention group patients received continuity of nursing. Effects were evaluated by comparing the two groups of patients or their family members with multiple options, such as wound care skills, therapeutic effect of wound, wound healing time, the compliance by wearing stretch hose pressure treatment, satisfaction to the effect of treatment of wound, the aspects to ET.Results60cases were visited in the whole experiment process in three months,30in the control group and30in the experimental group.(1) Patients or their family members in the experimental group were better at mastering wound care skills after applying the continuity nursing model than the control group (P<0.01).(2) In experimental group, wound healing time was33.89±4.55days, while in the control group, the time was43.67±9.99days. Therefore, the difference was statistically significant (T=-2.796, P=0.024). In experimental group,18cases cured,5with remarkable effect,4better, and3ineffective, While in the control group,9cases cured,3markedly effective,12better,6ineffective, it was obvious that the difference was statistically significant(Z=-2.570, P=0.010).(3) In the experimental group,29cases had good compliance of application of stretch hose completely,1with incomplete compliance, and the compliance rate of96.67%; While in the control group,18have good compliance in this regard,12with incomplete compliance, and the compliance rate of60%. Thus the difference was statistically significant(x2=11.882, P=0.001).(4) Between the experimental group and control group, differences of all the options investigated were statistically significant including ET support (z=-4.498, P<0.001), to wound effect(z=-3.254, P=0.001).Conclusion (1) With continuity of nursing, leg venous ulcer wound patients’and their families’care skills were improved significantly.(2) With continuity of nursing, leg venous ulcer wound patients have better healing status, shorter wound healing time.(3) With continuity of nursing, leg venous ulcer wound patients have better compliance of application of stretch hose.(4) With continuity of nursing, leg venous ulcer wound patients have better effective of wound healing, satisfaction to continuity of nursing service and credibility of ET.
