| Neonatal hypoxic-ischemic encephalopathy (HIE) is hypoxic-ischemic brain damage in perinatal period (hypoxic-ischemic, HI) resulting in neonatal hypoxic-ischemic brain damage (HIBD), which is the important cause of neonatal neurological system dysfunction. Perinatal cerebral hypoxic-ischemic can occur in the prenatal period, intrapartum and postpartum, which brings upon the growth of the newborn children stricken with encephalopathy such as cerebral palsy, epilepsy very serious impact, while increasing the pressures of family and society. With the advancing of medical science and the development of maternal health, perinatal care, the survival rate of premature births and children with low birth weight has increased significantly, but those improvement comes with high disability rate on preterm births and children with low birth weight. Therefore, effective treatment for neonatal hypoxic-ischemic brain injury is needed to be established as soon as possible, but how to treat neonatal hypoxic-ischemic encephalopathy still remains as a worldwide medical problem so far.Acupuncture treatment for neonatal hypoxic-ischemic brain injury is an simple and effective treatment which has achieved great development in the past20years.Especially In the treatment of cerebral palsy, epilepsy great breakthrough has been made. With the increasing treatment and researches on neonatal hypoxic-ischemic brain injury done by, some clinicians found that acupuncture treatment of children with encephalopathy and age of children receiving acupuncture treatment are closely linked, the sooner acupuncture practioners who grow in numbers. Some clinical practioners has found that there is a strong relationship between the effect and the age of children with neonatal HIE received acupuncture treatment. The sooner they receive acupuncture treatment the better the result can be. Thus there is great merit in research on acupuncture treatment effect in the neonatal period of HIE. The idea of acupuncture treatment in the neonatal period of HIE was born. Acupuncture treatment for HIE is a simple, effective non-toxic side-effects-free, etc. Differentiate and regulate children with HIE overall on the basis of the overall concept., play a role in the overall regulation. However, due to the insufficient experimental study on the mechanism of acupuncture treatment in HIE while consider the hazard and severity of HIE, Acupuncture treatment for HIE can only be applied under the premise of the establishment of a safe and effective method. Thus, experimental research on the mechanism of acupuncture treatment on animal HIE model has far-reaching significance on the promotion and expansion of acupuncture treatment for HIE. Modern acupuncture has summarized and innovated on the basis of traditional acupuncture, In the Lingnan region of China formed a "Jin three-needle" as the representative of a new genre of acupuncture, Jin’s team has been committed themself to the research of the encephalopathy in children in the past two decades, They has launched many clinical studies and achieved fruitful results, it is still a lack of research on the mechanism of acupuncture treatment for HIE. Based on the study of ancient and modern documents "Jin’s three needle technique" use points on the scalp and the time of intervention as entry(breach)point to explore how does acupuncture intervention and the time of the intervention effects the amino acid pathways on cerebral cortex of rats with Intrauterine fetal distress.PurposeEstablished rat model of HIE induce by Intrauterine fetal distress. Observe and track down weight changes, behavioral changes, amino acids (GLU, GABA) expression in the cerebral cortex of rats in normal group, model group, acupuncture intervention group, to explore the "Jin’s three-needles technique "on scalp points in the impact of different intervention time for HIE in neonatal rat brain microenvironment, providing the solid experimental evidence for acupuncture treatment of HIE and the necessary theoretical support for the clinical application of acupuncture treatment for HIE.Experimental Methods Experiment I:clasp the uterine artery on both side in pregnant rats with forceps, perform5min delay cesarean to mimic hypoxic-ischemic brain injury modeling rat with HIBD. Establish the normal group, sham operation group, the newborn rats in normal group are given birth by normal pregnant rats through normal labor, the newborn rats in sham-operation group are given birth through cesarean without clasping uterine artery by forceps. Observe changes in body weight, behavioral study and HE staining of brain tissue in the neonatal rats of normal group, sham operation group, model group.Experiment II:Neonatal rats from Experiment I are divided into normal group, sham operation group, model group, acupuncture group1, acupuncture group2in accordance with the principles of random assignment. Do not apply acupuncture on Model group, Apply acupuncture on acupuncture group1from14th days. Apply acupuncture on acupuncture group2from28th days. Acupuncture group1is divided into three group according to the principle of random assignment, execution on21th days(E21), execution on28th days(E28), execution on35th days(E35);Acupuncture group2is divided into three group according to the principle of random assignment, execution on35th days(E35), execution on42th days(E42), execution on49th days(E49). When each group has reach the observation time point, conduct behavioral assessment, execute the rats, extract brain tissue and use high-performance liquid chromatography to detect the expression Glu, GABA in the cerebral cortex. The experimentalresults1:A successful exploration to the method of modeling Intrauterine fetal distress induced HIBD rats. Providing definite modeling method for the study of neonatal HIE and conveniences to further follow-up research.Ⅱ:Effects of acupuncture on the expression of brain Glu:Indicators of acupuncture on mitochondrial pathway of apoptosis inhibition of expression of Glu, suppressing intracellular expression of Glu, exert protective effects on cells.Ⅲ:Effect of acupuncture on the expression of brain GABA:Acupuncture on indices of mitochondrial apoptosis pathway affect GABA, but the differences between the groups are not clear.ConclusionsAcupuncture in "Jin three-needle" scalp acupoints combined with dialectical point selection, to ensure treatment subject to a greater measure of improve the prognosis of hypoxic-ischemic brain damage, the mechanism maybe through the mitochondrial pathway of apoptosis, inhibiting expression of Glu and GABA, rather than protecting nerve cell function. |
