ObjectiveTo study the surgical treatment and prognosis of endovascular treatment andrelated factors of middle cerebral artery aneurysm.Material and methodA retrospective on arterial aneurysms September2008-March2014at the FirstAffiliated Hospital of Zhengzhou University, Department of Neurosurgery treatedwith complete medical records of252cases of brain analysis. Which surgicaltreatment of187cases of patients, endovascular treatment group were65cases.Contrast preclude different treatment modalities in patients taking preoperative state,HuntHess grading, head CT Fisher grade, history of hypertension, age,complications and discharge modified Rankin score level and other clinical data,using the chi-square test, t test and other statistical significant analytics methods.Results187cases of patients with surgical clipping group,162cases with goodprognosis after discharge, a good rate of85.7%,25cases of poor prognosis, poor rate of14.3%; endovascular treatment group of65patients, the hospital58caseswith good prognosis good rate of89.3%, a poor prognosis seven cases, the defectrate of10.7%. No patients were discharged in good prognosis rate and failure ratestatistically significant (chi-square test, p=0.297>0.05)? Poor prognosis rate at thesame level of preoperative Hunt-Hess grade patients was not statistically between thetwo treatment Learn the difference (p>0.05). Rate at the same level of poorprognosis in patients with preoperative Fisher grading between the two treatmentshad no significant difference (p>0.05). The same age group of patients with poorprognosis rate between the two treatments had no significant difference (p>0.05).History of hypertension had no effect on the prognosis of middle cerebral arteryaneurysm patients discharged. Two groups of patients in the postoperativevasospasm, arsine and then broken tumor, postoperative infection, postoperativehydrocephalus, there was no statistically significant difference (p>0.05) onpostoperative lumbar puncture rate. The average number of patients in the hospitaltwo days, the average operation time there was a significant difference (p <0.05) onaverage medical costs.ConclusionsThe microsurgical clipping and endovascular intervention can be effective intreating cerebral aneurysms. Endovascular surgery patients compared to patientstreated with surgical clipping group in a short time, rapid postoperative recovery,higher hospital costs. Two treatments in patients with good prognosis after hospitaldischarge rate, poor rate no significant difference in mortality. The same level ofpre-surgery CT Fisher grade and poor prognosis in patients with Hunt-Hess graderate was not statistically different between the two treatments. The same age groupwith poor prognosis of patients with no difference in rates between the twotreatments. History of hypertension had no effect on the rate of aneurysm goodprognosis.
