To evaluate the clinical Practicality and the further generalizability of this management system. To Understand clinical effects that traditional Chinese medicine have made on the chronic congestive heart failure disease management.Method:The Patients who have been diagnosed with chronic heart failure in the Guangdong Provincial hosPital of traditional Chinese medicine as manage objects. The200heart failure Patients discharged from hosPital are brought into randomly from January,2012to August,2012. The Patients discharged from Cardiovascular DePartment come from ER Sha Island HosPital, Fangcun District HosPital, Guangzhou camPus city hosPital have been established for the control grouP. The Patients discharged from Cardiovascular DePartment come from Guangdong Province General HosPital have been established for the administrative grouP.100cases are brought into the control grouP and100cases are brought into the administrative grouP, IncorPorate management follow-uP after24weeks. The CHF Patients fill in the Basic Information tables. The main contents of the Basic Information tables. include general information, vital signs, cardiac function.classification, related risk factors of cardiovascular diseases, Associated diseases. According to the rate scale to evaluate traditional chinese medicine symPtom score,western medicine symPtoms and signs score, and LiHFe life quality investigation score for included Patients.In order to determine the based score status for the included Patients. Administrative grouPs are managemented strictly by the management Practices and Processes of the integrated traditional and Western medicine management system of the chronic heart failure disease. which include many diseases management such as follow-uP, sPecialist outPatient clinics, SPecialist outPatient nursing, Self-management training classes, Doctor-Patient interaction, Patient-Patient interaction. The heart failure Patients in the control grouP make routine reture visits in SPecialist outPatient. The total efficiency of the administrative grouP is85.56%, the control grouP is69.47%, with a significant comParison (P<0.05). As the mortality rate, the administrative grouP is3.19%, the control grouP is5.56%, the difference was not statistically significant.Conclusion:the integrated traditional and Western medicine management system of the chronic heart failure disease Can imProve the quality of life of Patients, reduce the number of hosPitalized Patients and hosPitalization time, decrease hosPitalization exPenses.
