ObjectiveWe will analyze CYP3A5gene polymorphism of donor and recipient in children living donor liver transplant, and tacrolimus concentration/dose ratio after liver transplantation, to provide reliable reference index on the use of tacrolimus after liver transplantation, and optimize drug regimens and reduce adverse reactions.MethodsBlood samples were collected from donor and recipient of living donor liver transplant in our treatment group, The CYP3A5genotypes were determined by PCR-SSP method. The concentration/dose ratio were recorded after liver transplantation. According the genotype of recipient, the patients were divided into recipient expression group and recipient non-expression group. According to the genotype of donor, the patients were divided into donor expression group and donor non-expression group. According to the genotype of donor and recipient, the patients were divided into three groups:donor non-expression/recipient non-expression group, donor or recipient expression group, the donor expression/recipient expression group.ResultsThe CYP3A5genotypes of78patients were composed by47CYP3A5*3*3(60.3%),28CYP3A5*1*3(35.9%) and3CYP3A5*1*1(3.8%). The Co/D of Recipient expression group was significantly lower than recipient non-expression group in one week, two weeks, four weeks and eight weeks after liver transplantation. The Co/D of donor expression group was significantly lower than donor non-expression group in one week, two weeks and four weeks after liver transplantation. The Co/D of donor non-expression/recipient non-expression group was significantly lower than donor or recipient expression group in one week, two weeks, four weeks and eight weeks after liver transplantation. However, there was no significant difference between the Co/D of donor or recipient expression group and the Co/D of the donor expression/recipient expression group in one week, two weeks, four weeks and eight weeks after liver transplantation.ConclusionCYP3A5gene polymorphism is highly correlation with the individual differences of the dose of tacrolimus and blood trough concentrations after liver transplantation. The CYP3A5genetypes of donor and recipient have the same effect on Co/D ratio after children living donor liver transplantation.
