Objective:After major abdominal surgery in rabbits given intervention hydroxyethyl starch collaborative albumin and compared with control group and albumin group, By observing mesenteric microvascular leakage of albumin, albumin measurement of mesenteric microvascular leakage and leakage rate、Preoperative and postoperative detection serum albumin concentration and serum albumin concentration and serum concentrations of inflammatory mediators, To explore the hydroxyethyl starch collaborative albumin on rabbit mesenteric microvascular albumin leakage effects.Methods:The18Japanese rabbits were randomly divided into three groups, namely the control group (6), albumin group (6), collaborative (hydroxyethyl starch+albumin) group (6). Three groups of rabbits underwent surgery:gastrectomy plus pancreatic body and tail, splenectomy.after all rabbits were calories and other the nitrogen nutritional support, calorie150kcal from glucose and fat, respectively,70%of the heat,30%, nitrogen heat ratio of1:130. The control group was given Normal Saline25ml/Kg until death every day after surgery, Albumin group was given5%rabbit serum albumin25ml/Kg until death every day after surgery. Collaborative group (hydroxyethyl starch collaborative albumin) was given6%hydroxyethyl starch12.5ml/Kg and5%rabbit serum albumin12.5ml/Kg until death every day after surgery. Preoperative and postoperative detection point (48hours) detection serum albumin concentration and serum IL-6、TNF-α、VEGF、ICAM-1、HIS、IL-2concentration; after intravenous injection of rhodamine isothiocyanate the B labeled bovine serum albumin, laser confocal microscope dynamic observation of rabbit mesenteric microvascular albumin leakage, and to calculate the microvascular albumin permeability.Results:1. Variations of the Serum Albumin concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum albumin concentrations were lower, there was significant difference compared with preoperative(P<0.05).Between the two groups:①The control group, albumin group, cooperative group preoperative serum albumin concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group albumin concentration is higher, with the albumin group and control group were statistically significant (P<0.05).③Postoperative albumin group albumin concentration is higher, compared with the control group were Statistically significant(P<0.05).2. Variations of the Serum IL-6concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum IL-6concentrations were elevated, there was significant difference compared with preoperative(P<0.05).Between the two groups:①The control group, albumin group, cooperative group preoperative serum IL-6concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group serum IL-6concentration is lower, with the albumin group and control group were statistically significant (P<0.05).③Postoperative albumin group and control group, there was no significant difference in serum IL-6concentration(P>0.05). 3. Variations of the Serum TNF-a concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum TNF-a concentrations were elevated, there was significant difference compared with preoperative(P<0.05).Between the two groups:①The control group, albumin group, cooperative group preoperative serum TNF-a concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group serum TNF-a concentration is lower, with the albumin group and control group were statistically significant (P<0.05).③Postoperative albumin group and control group, there was no significant difference in serum TNF-a concentration(P>0.05).5. Variations of the Serum VEGF concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum VEGF concentrations were elevated, there was significant difference compared with preoperative(P<0.05). Between the two groups:①The control group, albumin group, cooperative group preoperative serum VEGF concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group serum VEGF concentration is lower, with the albumin group and control group were statistically significant (P<0.05).③Postoperative albumin group and control group, there was no significant difference in serum VEGF concentration(P>0.05).6. Variations of the Serum ICAM-1concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum ICAM-1concentrations were elevated, there was significant difference compared with preoperative(P<0.05).Between the two groups:①The control group, albumin group, cooperative group preoperative serum ICAM-1concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group serum ICAM-1concentration is lower, with the albumin group and control group were statistically significant (P<0.05).③Postoperative albumin group and control group, there was no significant difference in serum ICAM-1concentration(P>0.05).7. Variations of the Serum HIS concentrationComparison Group:The control group, albumin group, cooperative group Postoperative serum HIS concentrations were elevated, there was significant difference compared with preoperative(P<0.05).Between the two groups:①The control group, albumin group, cooperative group preoperative serum HIS concentration compared to the differences were not statistically(P>0.05).②Postoperative collaborative group serum HIS concentration is higher, with the albumin group and control group were statistically significant (P<0.05). ③Postoperative albumin group and control group, there was no significant difference in serum HIS concentration(P>0.05).8. Comparison of the rabbit mesenteric micro vascular leakage rate①Collaboration with the control group, between the albumin group, Albumin leakage rates were statistically different (P<0.05).②Albumin group and the control group, there were significant differences in the rate of albumin leakage (P<0.05).Conclusion:1. Rabbit after major abdominal surgery early(48h) existence the concentration of serum albumin declined, giving Hydroxyethyl starch collaborative albumin treatment can be reduced by lowering the magnitude of the early serum albumin.2. Rabbit after major abdominal surgery to give hydroxyethyl starch collaborative albumin treatment, can reduce early inflammatory mediators IL-6、TNF-α、IL-2、VEGF. ICAM-1increased range. The albumin treatment given alone had no effect on the early inflammatory mediators.Tips have anti-inflammatory effects of hydroxyethyl starch.3. Rabbit after major abdominal surgery to give hydroxyethyl starch collaborative albumin treatment, can reduce early rabbit mesenteric microvascular albumin leakage. and given alone albumin therapy may aggravate the early mesenteric microvascular albumin leakage.
