Comparison Of Quality Of Life And Its Related Factors In End-Stage Renal Disease Patients Of Different Renal Replacement Treatment

Objective To compare the quality of life of hemodialysis, peritoneal dialysis and kidney transplantation in end-stage renal disease patients,and to explore the related factors.Methods Adult patients with end stage renal disease receiving maintenance hemodialysis,Continuous Ambulatory Peritoneal Dialysis or kidney transplantations for more than3months in our hospital,the Second Affiliated Hospital of Kunming Medical University and Yunnan Kidney Disease Hospital between March2011and July2013were included in this study.There were160maintenance hemodialysis patients,52continuous ambulatory peritoneal dialysis patients and142patients with kidney transplantation.Kidney Disease Quality of Life Short Form (KDQOL-SFTM1.2) was used to evaluate the quality of life and the clinical and laboratory characteristics were also collected at the same time for each patients.Results1.Comparison of quality of life in three groups:①The total score of SF-36and its eight dimensions, the total score of KDTA and the scores of its several dimensions including SPL,EKD,BKD and SexF were all significantly higher in kidney transplantation group than in hemodialysis or peritoneal dialysis group(p<0.05); while the scores of WS dimension in kidney transplantation group were significantly higher than in hemodialysis group(p<0.05),and the scores of Sleep,Sos dimensions in kidney transplantation group were significantly higher than in peritoneal dialysis group(p<0.05);②Quality of life of hemodialysis group was significantly higher than peritoneal dialysis group in Sleep and Sos dimensions,and lower than peritoneal dialysis group in Energ,EKD and WS dimensions(p<0.05);③The scores of physical health and mental health were significantly higher in kidney transplantation group than in hemodialysis and peritoneal dialysis group(p<0.05),but the scores of hemodialysis and peritoneal dialysis group had no significant differences in these two dimensions.2.demographic indicators and quality of life:①omparison of general data:The gender, age,ethnicity, marital status and education level composition of three groups had no significant differences (p>0.05);②emographic data and the total score of SF-36:HD group:The total score of SF-36was significantly different in subgroups of age(p<0.05); PD group:The total score of SF-36had no significant differences in subgroups of gender, age,ethnicity, marital status and education level;KT group:The total score of SF-36was significantly different in subgroups of age and marital status (p<0.05);③emographic data and the total score of KDTA:The total score of KDTA in hemodialysis and peritoneal dialysis group had no significant differences in subgroups of gender,age,ethnicity, marital status and education level(p>0.05);KT group:The total score of KDTA was significantly different in subgroups of marital status and education level (p<0.05);3.Correlation analysis:①he scores of SF-36and KDTA in hemodialysis groups and the scores of SF-36in peritoneal dialysis group were negatively correlated with age,while the scores of SF-36in kidney transplantation group were positively correlated with age;②The scores of SF-36in three groups were all positively correlated with the level of hemoglobin;③The scores of SF-36and KDTA in hemodialysis groups and kidney transplantation group and the scores of SF-36in peritoneal dialysis group were positively correlated with the level of serum albumin;④The scores of SF-36in hemodialysis groups and the scores of SF-36and KDTA in kidney transplantation group were positively associated with education level;⑤The scores of SF-36and KDTA in kidney transplantation group were negatively correlated with the level of serum creatinine.Conclusion The patients with kidney transplantation scored higher in the SF-36and its all dimensions and KDTA and its several dimensions than hemodialysis patients and peritoneal dialysis patients;Quality of life of hemodialysis group was significantly higher than peritoneal dialysis group in Sleep and Sos dimensions,and lower than peritoneal dialysis group in Energ,EKD and WS dimensions;The overall quality of life of patients with kidney transplantation is higher than that of hemodialysis and peritoneal dialysis patients.The quality of life of patients in three groups is correlated with age,education level,serum albumin,hemoglobin and other factors.In order to improve the patients’ quality of life,we should pay more attention to these indicators in clinical work.
