Objective:1.Tentatively establish the model of rabbit obstructive azoospermia2.Explore the feasibility and operational details of microscopic vertical two-needles tube nested type vas deferens epididymis anastomosis of animal experiments Thereby provide a simple, safe and feasible experimental platform for male doctors on the skill training of Microscopic vas deferens epididymis anastomosis.3.Through animal experiments,we will consolidate and master the though and skills of Microscopic vas deferens epididymis anastomosis.Methods:Selecting38healthy male California5-6months big white rabbits as experimental object on the basis of months of age, weight and semen analysis. Ligature bilateral vas deferens near the epididymis respectively under the1%pentobarbital vein anesthesia induction, in order to establish rabbit obstructed no refined disease model.Check the semen after2months,If there is no sperm,we will try to carry out the operation of2-suture longitudinal intussusception vasoepididymostomy on the rabbits,in1%pentobarbital intravenous anesthesia combined5%lidocaine under local anesthesia.2-4months after the operation,collect semen for each male rabbit3times a month,interval for3days every time.continuous take semen for3months,after that we will finish the experiment.Semen test first every time,if there is sperm found,Continue to use the SCA semen analyzer analysis the semen,and record the main semen parameters;if no sperm found,check the semen at high power field after centrifuging the semen,Record the final result.Results:1.In the experiment of building rabbit obstructed no refined model,one rabbit died of epididymis testicular necrosis and wound infection,another died of sudden cardiac arrest because of anesthetic injecting too fast,The establishment of the obstruction model of the others went well.The average time of operation was 60min±8min,The average recovery time was72±6h,None of the rabbits had infection、fever nor diarrhea,and so on.2.In the part of microscopic anastomosis,2pabbits underwent Unilateral microscopic anastomosis because of adhesion serious of the Local tissue.The experiment of the others went smoothly.The average time of operation was140±10min,The average recovery time was120±6h.3.2-4months after the operation we conducted the semen analysis combine with microscopic test after centrifugation.The results showed that,there were8detected sperm in the second month after the operation,12in the third month,and just4in the forth month.A total of23male rabbit detected in semen sperm,among that,there were4just detected sperm but had no semen analysis.The rate of repatancy in this study was63.89%.To the19rabbits which had the results of semen analysis,compared with the preoperative, there were no difference between the quantity of semen (P>0.05), but the sperm density, living rate, activity had statistically significant (P<0.05)Conclusion:1.We conducted vasectomy near epididymis by means of incision (about1cm) near the bottom of the scrotum incision to establish rabbit obstructed no refined disease model.and we found the rabbits bad small local damage in the operation,fewer complications and fast postoperative recover,The operation was simple and feasible.2months later.when we conducted the microscopic anastomosis,Under the naked eye we found that the rabbits’s epididymis volume increased epididymis pipe were full.Under the microscope vision,we could see epididymis pipe(about0.4-0.5cm)、spermaduct、blood vessel and other organization clear, the operation became easy and went well.This study confirmed the scientific nature of the project design---the rabbits’s testicle and epididymis are bigger,epididymis pipe and vas deferens are coarser,these characteristics are more close with people,the operation is easy and it has the advantage of that we can take semen over and over again.In a word,rabbit is a good experimental simulation object to microsurgical anastomosis.Through the results of the postoperative semen test,the postoperative repatancy rate of this experiment was63.89%.it exactly confirmed microsurgical vas deferens epididymis tube anastomosis about rabbit was feasible.And had the characteristics of minimally invasive, safe and easy to operate,it can be used as a very good skills training method and model for male clinical surgeons before performing microsurgical vas deferens epididymis tube anastomosis.This was worthy to research and generalizeIn addition,through the study we found that some main semen indexes changed before and after the experiment,however,we though detecting rabbit’s sperm after operation as succes,so for the above differences,we do no further research about that.Through the study, we more consolidated and mastered the though and operation skills of microsurgical vas deferens anastomosis,and we really realized microscopic anastomosis surgery ws a fine, long time operation,it asked the performer to have solid microsurgical basis and enough patience.Microscopic skill levels of the performer was crucial for the success of surgery,therefore, as a male surgeon,we should pay attention to the development of micro thinking and microscopic basic skill training,in order to serve the clinical better.
