Objective:To evaluate the hemodynamic changes of brain in lesion areas ofMoya-moya patients,and investigate the diagnostic value of MR perfusion-weightedimaging (PWI) in patients with Moya-moya Disease.Methods: MR PWI were performed in5patients with unilateral Moya-moyaDisease diagnosed by DSA,and all of the patients accepted CTA research at first,DSAresearch were used to clarify diagnosis at last. The Position and size of scanning wassame to T2weighted imaging, using the free attenuation EPI sequence, scanning50times in a row.At the6th collection, paramagnetic contrast agent Gd-DTPA wereinjected by high pressure syringe from elbow vein.The amount of contrast agentinjected was0.2mmol per kilogram of body weight, the total dose was injected in4s,then immediately followed by the same volume injection of saline. MR-PWI data wereprocessed on a GE workstation with a Functional software package, got the timeintensity curve,worded out the cerebral perfusion parameters.Choose the largest levelwhich can show the lesion best, and marked out the abnormal perfusion areas in thefigure, made the contralateral as relatively normal reference, measured the cerebralperfusion parameters of bilateral cerebral hemisphere.This research used the cerebellum as a reference to standardized all perfusion parameter values. All data was processed bySPSS11.0software, rTTP, rMTT, rCBF and rCBV values were followed by matching ttest, P <0.05significant for differences.Results:The abnormal perfusion findings were depicted in all patients withmoya-moya disease. The abnormal findings of PWI were delayed TTP and rMTT,normal or slightly decreased rCBF. The rCBV mean values of all the patient’s lesionside were1.13±0.28,had a significant for differences compared with thecontralateral(P <0.01); The rCBF mean values of lesion side were1.11±0.36,andhad statistically significant difference compared with the contralateral(P <0.01).Conclusion:PWI can assess the degree and scope of cerebral ischemia bydisplaying the cerebral hemodynamics in patients with MMD,and improve the diagnosissensitivity and specificity of MMD.The evaluation of Vascular and perfusion in patientswith MMD is important to perioperative preoperative and postoperative prognosisprediction.
