| Objective:To compare the clinical effects of total hip replacement and femoral headreplacement in the treatment of femoral neck fractures in65years-80years patients.Methods:Retrospective analysis of January2003to April2013admitted65years-80years43patients with femoral neck fractures,15males and28females,average73.4years. In typing of fracture classification,there are31cases of under head type,9casesof jugular type,3cases of basal type.Among them,39cases with medical problems.29cases with two and two more medical problems.There are18cases in Group total hipreplacement.And there are25cases in Group femoral head replacement.In the THAGroup,there are8males and10females.In the HA Group,there are7males and18females.Recorded the hospitalization period,operation time, intra-operative bloodloses,postoperative drainage,hip functional score and postoperative complications,tocompare by statistical analysis.Result:43cases patients were1years to6years follow-up.Femoral head replacementgroup is lower than the total hip replacement group (P <0.05) in operation time,intra-operative blood loses.There is no significant difference between HA group andTHA group in hospitalization period,postoperative drainage(P>0.05).There is nosignificant difference between HA group and THA group in the Harris score after1year(P>0.05). There is a significant difference between HA group and THA group inthe Harris score after5years(P>0.05).THA group is superior to the HA group.Femoralhead replacement group was obviously lower than total hip replacement group(P <0.05)in the long-term complications.Postoperative complications included,acetabulum wear,prosthetic aseptic loosening, artificial prosthesis dislocation, and prosthesis infection.Conclusion:The effective treatment of femoral neck fracture in old patients is total hip replacement and femoral head replacement. Hip replacement can significantly reducethe incidence of postoperative pain in patients.Patients early weight-bearing walk tokeep the limb muscle strength.It is important for restoring good body health.There is nosignificant difference between HA group and THA group in hospitalizationperiod,postoperative drainage,early postoperative complications and early hip functionalscore.There is a significant difference between HA group and THA group in operationtime, intra-operative blood loses. The total hip replacement is higher than femoral headreplacement in operation risk. The total hip replacement is better than femoral headreplacement in the postoperative long-term Harris score and long-term complications.So the total hip replacement is good for better health conditions,better tolerancepatients.Though HA group is superior to the THA group in operation time,intra-operative blood loses,HA group is worse than THA group in long-termoutcomes.So femoral head replacement is good for worse health conditions,smallerexercise-load patients. |
