Correlation Study Of Intima-media Thickness Of Carotid Artery, High Sensitivity C Reactive Protein, Myeloperoxidase, Bilirubin And Thyroid Function In Patients With Coronary Heart Disease

Objective:To investigate the relationship among the High-sensitivity C-reactiveprotein(Hs-CRP),myeloperoxidase(MPO),total bilirubin (TB),thyroid function andcarotid artery intima-media thickness(IMT),in order to assess coronary heart disease(CHD) risk populations severity of atherosclerosis, coronary angiography indicationsand treatment evaluation guide provides a reliable indicator.Method:Totally88patients from the Second Affiliated Hospital of DalianMedical University the with chest pain and the tentative diagnose is coronary heartdisease. Blood samples were collected from cubital veins after fasting8hours. Testbiochemical criterion including cholesterol (TC), low-density lipoprotein cholesterol(LDL-C), high density lipoprotein cholesterol (HDL-C), triglycerides (TG), uric acid(UA), high-sensitivity C-reactive protein (Hs-CRP), myeloperoxidase (MPO), totalbilirubin (TB), free triiodothyronine three (FT3), free four triiodothyronine (FT4) andthyroid stimulating hormone (TSH).All patients received coronary angiography, twoor more experts to estimate the coronary artery, including left main coronary artery(LM),left anterior descending artery (LAD),circumflex artery (LCX),right coronaryartery (RCA).According to the results of coronary angiography is divided into twogroups:1)coronary heart disease(CHD) group: total of64cases (including30males,34females).The coronary heart disease group will be divided into single-vessel disease(total of26cases,including11males and15females)double-vessel disease(total of26 cases,including11males and15females) and three-vessel group(total17cases,including9males and8females)in which the involvement of the left mainstenosis≥50%is counted as double-vessel disease,more than double counted asthree-vessel disease lesions;2)control group:total24cases were enrolledcases(including13males and11females).Compare with the several groups,Hs-CRP,MPO, TB, TSH, FT3, FT4, carotid IMT and coronary angiography. Analyzed therelationship among Hs-CRP, MPO, TB, TSH, FT3, FT4and carotid IMT.RESULT:1.The three index(smoking,drinking,sex)showed no statisticallysignificant in control group, single-vessel disease group, two-vessel group andthree-vessel group (P>0.05);The two index(age,TG,UA))showed no statisticallysignificant in single-vessel group,two-and three-vessel group in controlgroup,between single-vessel group and three-vessel group.compared with the controlgroup, the TC and LDL-C are higher in in single-vessel group and three-vesselgroup(P<0.05);compared with the control group, the HDL-C levels are lower insingle-vessel group and three-vessel group(P<0.05);2.Compared with the control group, IMT increased obviously in two andthree-vessel group, the difference was statistically significant (P<0.05; P<0.01);compared with single-vessel group, IMT increased significantly two-vessel group,three-vessel group, the difference was statistically significant (P<0.01). At the sametime, compared with control group, The severe coronary artery disease,Hs-CRP, MPO,STSH index increased significantly in single-vessel, two and three-vessel group with,the difference was statistically significant (P <0.05),compared with the control group,the TB index decreased in single-vessel, two and three-vessel group with coronarydisease aggravated.the difference was statistically significant.(P<0.01). But there isno significant difference in FT4, FT3;3.IMT and Gensini score was significantly positively correlated, the differencewas statistically significant;4.Analysis the Hs-CRP, TB, MPO, STSH, FT3, FT4and carotid artery IMT ofCHD group show: Hs-CRP, MPO, STSH and carotid artery IMT was positivelyrelated, the difference was statistically significant (P<0.01), TB was negatively correlated with IMT of carotid artery, the difference was statistically significant (P<0.05) FT4, FT3, and no significant correlation between carotid artery IMT;5.in CHD group, the IMT as the dependent variable, Hs-CRP, MPO, STSH, TBas dependent variable, the multiple linear stepwise regression analysis. The resultsshowed that entered the regression equation were Hs-crp, MPO, TB, Hs-CRP,suggesting that high MPO level and low TB may be independent risk factors for CHDpatients with high IMT. While the Hs-CRP standardized regression coefficient was0.414, suggesting that the effect of Hs-CRP on carotid artery IMT and larger, TSH didnot enter into the equation, suggesting that TSH may not be an independent risk factorfor IMT disease.Conclusion:1.patients with coronary heart disease, IMT value increasedgradually along with the severity of coronary artery lesions with positive correlation,(P <0.05) there is significant correlation.2.In patients with coronary heart disease, Hs-CRP, MPO, TSH and the severityof coronary artery lesions were positively correlated, negatively correlated with TBlevels and the severity of coronary artery disease, FT3, FT4and coronary arterylesions have no obvious correlation.3.In patients with coronary artery disease, carotid IMT, MPO, and Hs-CRP werepositively related to TSH, IMT was negatively correlated with TB, FT3, and FT4haveno obvious correlation.4.Hs-CRP, MPO level and the lower TB level may be the independent riskfactors for CHD with high IMT. Effect of Hs-CRP on carotid artery IMT is big,independent risk factors of TSH may not be IMT lesions. The combination of multifactors, has higher predictive value for CHD.
