Objective:To investigate the Burkholderia cepacia infection status and the antimicrobial resistanceof Burkholderia cepacia from August,2009to December2013by understanding the sources of speciman in our hospital to analyze the risk factor. of the Burkholderia cepacia infection.Probe the clinical characteristics of pulmonary infection.to direct reasonable utilization of clinical antibacterials.Methods:150strains of Burkholderia cepacia from the Second hospital of Shanxi Medical University between August2009and December2013were collected as a retrospective cohort study and analyzing the antimicrobial resistance of150strains of Burkholderia cepacia to Minocycline, Trimethoprim+Sulfamethoxazole, Ceftazidime and Meropenem.Patients’sexuality, age, severity of illness, length of hospitalization, number/types of indwelling catheters, concomitant infections and prior antibiotic exposure of112patients with positive cultures of Burkholderia cepacia isolated from respiratory tract、blood、urine and stool et.al. To assesse severities of illness by the APACHE II (acute physiology and chronic health evaluation II) score. T test was used to determine the clinical significance of positive cultures of Burkholderia cepacia isolated from respiratory tract blood,urine and stool et.al,and approach effective factors of the outcomes.Results:150strains of onion burkholderia bacteria has126strains from sputum specimens,10drainage fluid and secretion specimens,6blood specimens,2central venous catheter specimens,2urine specimen and2throat swab specimen;The sensitive rate of meropenem, imipenem, selectrintrimesulf, minocycline, ceftazidime respectively were65.66%,14.28%,76%,68.09%,74.11%; The investigating cases show that150strains of onion burkholderia bacteria isolated from112patients, and the patients are with more than two kinds of basic disease, merging with other bacterial infection is89people(79.46%),92people of indwelling catheter (82.14%), all patients before cultivating the pathogenic bacteria used antimicrobial agents, among them,61cases used carbape em antibiotics. Among them,73patients used cephalosporin;61patients used the carbapenem antibiotic;47patients used antifungal drugs;33cases of patients with the anti bacteria medicine;31patients using the enzyme inhibitor drugs;28patients used fluoroquinolones; others include broad-spectrum penicillin antibiotics, macrolides, tetracyclines, anti anaerobic antibiotics fungi etc.. Average length of stay is31±24days before cultivating the pathogenic bacteria, and of which112cases,65cases have improved (58.04%),22cases died (19.64%), giving up treatment is8cases (7.14%), the cure is17cases (15.18%).Conclusion:①The susceptible population of Burkholderia cepacia infection was older person, especially older man. Lower respiratory tract was the most common position of Burkholderia cepacia infection, it also can cause blood, urinary and alimentary system infection.②incision of trachea or (and) tracheal intubation, gastric tube detained, drugs of inhibiting gastric acid secretion, deep vein tube detained, length of using antibiotic drugs is long and using antibiotic drugs more than two types may be the risk factors for Burkholderia cepacia infection.③Using the APACHE criteria to evaluate the infection of onion and Burke Holder bacteria in hospitalized patients with serious illness severity. When the APACHE II score≥20points, the risk of death in patients with increased significantly.④Trimethoprim+Sulfamethoxazole and Ceftazidime are the optimal antibiotics of choice for the treatment of Burkholderia cepacia infections in our hospital.
