Objective:To assess the clinical effect and radiological results of two methods of anterior cervicaldecompression and reconstruction for the treatment of multilevel cervical myelopathy.Participants:Retrospective analysis the cases of the patients with anterior cervical multilevelmyelopathy with titanium mesh and locking plate system for treatment in FuzhouGeneral Hospital from March2009to March2013,54patients were follow-up.Therewere32male patients and22female patients,whose average age was58-year-old.12to48monthes were followed,averaged29.8monthes.Methods:Forty two patients with continuous multilevel cervical myelopathy treated by the sameteam were divided into two groups.Group1was performed the two segmentcorpectomy and reconstructed with a long titanium mesh and plate,Group2wasperformed the combination with corpectomy and discectomy.The parameters includedintervertebral height,JOA score,D numerical value measured by the tools of PACSsystem used in our hospital were evaluated Lordosis of the fusion and lordosis of thecervical spine respectively.Adjacent segment degeneration was diagnosed based onthe hyperplasia of adjacent segment.Statistic analysis were used to evaluate by SPSS19.0.Results:JOA score in all group s was improved post operation,but there was no significantdifference among the two groups.The post operation intervertebral height lose of group1was more significant than the other group,seven cases of titanium meshes subsidencehappened in group1and eight same case happened in group2.Conclusion:All the two surgical methods of anterior decompression and fusion can procurewonderful clinical effect,but group2were better than group1in the stability and themaintenance of intervertebral height post operation,either in the degeneration ofadjacent segment.
