The Effect Of Clinical Research With Enterprise Stent-assisted Coil In Endovascular Therapy Of Intracranial Wide-necked Aneurysms

0bjective:Through a retrospective clinical histories study of the wide-neck Intractableintracranial aneurysms respectively treated with Stent-assisted coil(Enterprise)and simplecoil or ballon with coil, to compare the curative effect,the aneurysm again breakingincidence and the satisfaction rate between two groups, and summarize the experience ofStent-assisted coil treatment that how to improve the clinical efficacy of treatment, reducethe intraoperative and postoperative complications, increase the cure rate and the quality ofsurvival patients suffer from Intractable intracranial aneurysms.Methods:113cases with intractable wide-neck cerebral aneurysms were recruited fromThe First Affiliated Hospital of ZUNYI Medical University and The Third AffiliatedHospital of ZUNYI Medical University between2009.7to2013.07.And54cases weretreated with Stent-assisted coil (Hunt＆Hess ClassificationⅠ11，Ⅱ27，Ⅲ14，Ⅳ2)，whileoher59cases were treated with only coil or ballon and coil (Hunt＆HessClassificationⅠ16， Ⅱ25， Ⅲ18). Actively follow-up3to6monthsrespectively,Statistically contrastive We analyze and contrast the cure rate, theintraoperative and postoperative complications, and the satisfaction rate between the towgroups through Statistical analysis.Results:1. Clinical effect (the effect of embolization): In the Stent-assisted coil(Enterprise)group of54cases,54cases show aneurysm density filling (almost100%),0cases showmost of the stuffing (90%),0patients show contrast agents stranded by postoperativeangiography; in the single coils group of59cases,56cases show aneurysm density filling(almost100%),2cases show most of the stuffing (90%),1patients show contrast agentsstranded in postoperative angiography. All of them have significant difference (χ2=4.26, P=0.04, which shows that the embolization effect of stent-assisted coil group wasobviously better than the pure coils. 2． Intraoperative complications: The Stent-assisted coil(Enterprise) treatment groupcompared with the pure coils or ballon and coil treatment group, the incidence ofintraoperative rupture again were1.8%and3.3%, the displacement of the coil, coil out andcoil unwinding rate were0%and3.3%, the incidence of intraoperative cerebral embolismor cerebral thrombosis events were3.7%and0%, the incidence of cerebral vasospasmwere3.7%and1.6%, all of them have significant difference (χ2=4.60, P=0.04),which shows that the intraoperative complications of stent-assisted coil group weredecreased significantly compared with the pure coils。3.satisfaction rate of postoperative: Tracked3-6months and assessed with modifiedRankin scale (MRS)(0~2means satisfaction;3~5means satisfied),The satisfaction rateof the stent-assisted coil group is93.02%,while the pure coils or ballon-assisted coilgroup is76.47%. It has significant differences (χ2=4.60, P=0.03), which shows thatthe prognosis effect of stent-assisted coil group was better than the pure coils orballon-assisted coil.Conclusions:1Intractable wide-neck intracranial aneurysms respectively treated with Stent-assistedcoil(Enterprise) can increase the cure rate， especially the wide-neck， reduce theintraoperative complications, and improve the quality of survival patients.2Intractable wide-neck intracranial aneurysms respectively treated with Stent-assistedcoil(Enterprise) is a kind of effective,and safe therapeutic method.
