Obiective The incidence of lower extremity arteriosclerosis obliterans which is major cause of Lower limb atherosclerosis and diabetes in our country is getting higher and higher. It seriously affect the life quality of middle-aged and elderly people.The imaging examination method for lower extremity atherosclerosis have a lot, The digital subtraction angiography(DSA) is considered the "gold standard", However, some disadvantages of this method, includinginvasiveness, risk of complications.Since the advent of multi-slice spiral computed tomography (MSCT), due to its safety, rapid, noninvasive and high accuracy, computed tomography angiography (CTA) has been widely used in clinical. Now retrospectively analyzed15cases of lower extremity atherosclerosis patients with CTA examination, all of them had DSA examination within two weeks, to explore the value of multi-slice spiral computed tomography angiography(MSCTA) in the diagnosis of artherosclerosis in lower extremities.Methods:The15cases of patients with lower extremity arteriosclerosis obliterans were received64-slice spiral CTA scan, all of them received DSA examination of both lower extremities within two weeks. Each patient’s lower extremities vessels is divided into22segment. Artery stenosis is divided into five levels of normal, mild, moderate, severe stenosis and occlusion,and with DSA for evaluation standard. By statistical analysis software SPSS17.0, processing the results of DSA and CTA two checking method, we get two-way orderly and the same as the columns of the league table material analysis. By the Kappa consistency inspection processing the results of CTA and DSA,we get Kappa value and P value. For the gold standard with DSA, we evaluation the diagnosis efficiency of CTA by which detection lower limbs of narrow disease (sensitivity, specific degrees, accuracy, the positie predictie alue and negatie predictie alue).Results CT angiography (CTA) contrasting the "gold standard" DSA have good consistency and high efficiency of the diagnosis in the lower limbs imaging artery occlusion disease.In the assessment of the narrow classification, the sensitivity of mild, moderate, severe narrow narrow narrow and totally occluded were99.2%,97.5%,95.3%, and100%, the respectively degrees of them were96.5%,96.1%,96.5%,98.7%and the accuracy of them were98.2%,97.6%,96.2%,98.2%.Conclusion1ã€CT angiography (CTA) can accurately assess the degree of stenosis of the lower extremity vascular occlusive disease, and accurate display the range, quantity, degree of lesions.2ã€CT angiography (CTA) and the post-processing imaging is a minimally invasive, quick and safe inspection method, and can be used for lower limb artery occlusion of the disease first screening method. |
