The specific society, institution, economy, politics and other factors of Japan gave birth to its long-term care insurance system. Its establishment is influenced by the aging of the population, the continuous rising demand for long-term care, the weakening function of domestic longterm care, the highlighting problems of the elderly welfare system and elderly care system, the distress of social security, the long run of slump, the feminist movement and the Party Game. By analyzing Japan’s long-term care insurance system with the Gilbert social welfare policy analysis structure, we find the system design seeks to be sophisticated, on both the basis or content of its distribution and service delivery or raise funds. Through several constantly updated and improved reforms, the design of the system also showed the development and application of aging in place, regionalism and diversified welfare concept in Japan. During the past fifteen years, the system has played a vital role in not only solving the social long-term care problems to a large extent, improving the Japanese social security system for the elderly, realizing the goal to support the aging society with the joint efforts of the whole society, but also promoting development of the long-term care industry and creating new jobs. However, challenges exist: the weakness of financial sustainability, the low level of allopatric overall planning, the insufficient supply of service, the heavy burden of domestic long-term care, the shortage of manpower and so on. With the accelerated aging process and the climbing demand of long-term care, establishing a long-term care insurance system is a crucial choice for China to face social pension risks. In view of the experiences of Japan, the government must accelerate the developing pace to create an environment that benefits China’s long-term care insurance system with concise system design, wide covering group, clarified insurance content and way of payment, focus on accessibility of service delivery, focus on available service and sustainable financing mechanism.
