Community - - Research Home Care Situation And Build Self-managed Hospitals Stoma Patients

Objective:To investigate the self-management status discharged from the hospital within a year and the factors that affected the ability of self-management of ostomy patients; to construct hospital-community-family nursing model for the self-management status of enterostomy patients, and to provide references for enterostomy nursing care.Methods:Measured the reliability and validity of self-made enterostomy patients self-management questionnaire, and investigated the62ostomy patients’ self-management status discharged from the hospital within a year and explored the factors that affected the ability of self-management. Based on the literature review and the expert interviews,to construct the basic framework of hospital-community-family nursing model,33experts were consulted twice by using the Delphi method. Applicating SPSS17.0statistical software for data processing.Results:The Cronbach’a coefficient of enterostomy patients self-management questionnaire was0.916,the content validity index was0.903. The overall score of the self-management of ostomy patients was92.45±12.87, Stomy care management achieved the score (30.16±3.62), daily life management got (26.48±4.02), information management score was (13.44±3.32), symptom management got (11.77±3.17), and psychological management was (10.53±2.50). Multiple stepwise regression analysis showed that age, culture level and colostomy acceptance was important influence factors of ostomy patients self management. Got the hospital-community-family nursing model consists of objectives, implementing agencies,organizational structure, nurse responsibilities and job contents, totaling22items. Response rates of two rounds of expert consultation were94.29%and100%, respectively. The authority coefficient was0.88, Kendall’s W were0.20and0.22, respectively. The Kendall concordance coefficient of significance test P<0.05. The results have statistical significance.Conclution:The enterostomy patients self-management questionnaire developed in the study has good reliability and validity. Self-management status of ostomy patients was the medium level, age, culture level and colostomy acceptance was important influence factors of ostomy patients self management. This suggestsed the nursing staff should strengthen nursing intervention to the person who are old、low degree and unable to accept ostomy. Experts had good initiative and representation in the construction of hospital-community-family nursing model for the enterostomy patients, it could provide the scientific method for the ostomy nursing.
