Objective: The distal radial fracture is one of the most frequent fracture in theorthopedic clinic. Its incidence rate accounts for 6.57% of system bones fractures.Middle-aged and old-aged woman is susceptible to it. With the appearance of the problemsuch as the development of traffic, the incidence rate of the distal radial fracture will behigher and the type will be more complex. The instable distal radial fracture is usuallyconsidered to be treated by surgical therapy, but because of the national economicdevelopment is not enough to make the entire people get free treatment, and some patients’condition is not allowed to be operated. While in TCM orthopedic way, reductionmanipulation and splint immobility is proved to be quite useful, so, in this article, aninvestigation was conducted in order to study and compare the clinical effect of Splintimmobility and external fixator on the patients with instable distal radial fracture; we alsocompare the Medical Economics Evaluation, in order to provide some proves for thepatients to choose.Methods: This article reviewed the knowledge of distal radial fracture in bothancient literature and modern medical research systematically. Summarizing the etiology,pathology, medicine of FDR in TCM; Describing the development and tenet of reductionmanipulation and splint immobility; Suming up the development of the anatomy, reason ofinjury, research of biological mechanism, diagnosis and treatment. 45 patients whosuffering from instable distal radial fracture were collected. We divide these patients intotwo groups. One group (22cases) were treated by Splint immobility, the other (23cases)were treated by external fixator. Both group were observed ten weeks. Every patient wasevaluated by PRWE wrist evaluate form, Lidstrom X-ray evaluate form and MedicalEconomics EvaluationResult: Through clinical research, the result shows that difference is obviousbetween two groups in anatomic structure of wrist and lose of the reduction (P<0.01), thefixator-group is superior; About easing the pain, fixator-group is better than the splint-groupthe day after the treatment (P<0.05), but in the 10th week,the fixator-group is better(P<0.05); However, there is no significant difference between two groups in function in oneweek after restoration, in the 5th week,we found that the splint-group is better, in the 10th week,there’s no difference again, but the reduction evaluate is clear(P<0.05), it stands thatthe recovery of function in fixator-group is better between the last 5 weeks; Last, throughthe Medical Economics Evaluation, we found that the splint-group is high superior than thefixator-group(P<0.01).
