Objects: To investigate the clinical characteristics of pinealoma, analyze and evaluate the clinical efficacy of modified Poppen approach in the treatment of pinealoma, so as to deepen our understanding and improve the treatment efficacy against this disease.Methods:22patients with pinealoma receiving surgery treatment in the department of Neurosurgery, the first affiliated hospital of dalian medical university from July2005to Aug2013were selected as our study subjects. The clinical data, surgery methods and early follow-up results of the22patients with pinealoma receiving treatment with modified Poppen approach were retrospectively analyzed and KPS scores pre-and post operation were compared. Data was analyzed with SPSS19.0software package.Results: Of the22cases,18were totally resected and4were sub-totally resected; pathological results after surgery indicated germ cell tumor in8cases, pineal cell tumors in6cases, cholesteatoma in3cases, pineal tumor in2cases, benign meningioma in2cases and metastatic carcinoma in1cases; good recovery was observed for all patients after surgery and intracranial hypertension was disappeared, as well as improved coordinate movement. Postoperative worsening of condition was observed in1case and recurrence of disease was observed in3cases who gave up treatment afterwards and no death case was observed. Pre-and postoperative KPS score comparison showed significant difference.Conclusion:Modified Poppen approach is an effective surgery method and an ideal approach for the treatment of pinealoma. It also provides more vision field during surgery and protects deep cerebral veins, makes treatment of blood supply artery more easily and reduces prolonged brain tissue traction, such as occipital lobe, and unnecessary collateral damage which is especially suitable for pediatric patients and larger tumors.
