| ObjectiveTo investigate the changes of fasting serum Orexin A and Nesfatin-1levels and theirroles in patients of newly-diagnosed polycystic ovary syndrome(PCOS).Methods1.57patients and56controls were selected and each group was divided into over-weight/obese and nonï¹£obese subgroups according to body mass index (BMI)≥25kg/m2or<25kg/m2. PCOS group was divided into PCOS-OW/OB subgroup(n=32) and PCOS-NW subgroup (n=25)ï¼›and the control group was divided intoControl-OW/OB subgroup (n=26) and Control-NW subgroup (n=30).2.Collecting name,age,menstrual cycle and delivery history such basic information ofall the objectives by questionnaire survey. Anthropometric measurements includingheight,weight,waist circumference(WC) and hip circumference,BMI and waist-hipratio(WHR) were calculated. Simultaneously complete the physical examination.3.Blood samples were collected in follicular phase and were used for testing theconcentrations of luteinizing hormone (LH), follicle-stimulating hormone (FSH),testosterone (T), triglyceride(TG), total cholesterol(TC), high-density lipoprotein (HDL),low-density lipoprotein (LDL) in all the subjects.4.All the subjects experienced a75g oral glucose tolerance test after12-14hoursovernight fasted. Blood samples were collected in OGTT-0min, OGTT-30min,OGTT-60min and OGTT-120min, used for measuring the concentrations of glucose,insulin, OXA and Nesfatin-1. Then HOMA-IR was calculated..Results1. Comparisons of basal data and clinical parameters:There were no difference in age,BMI, WC, TG, TC, HDL, LDL, P2HBG between PCOS group and Control group. WHR,FBG were higher(P<0.05),and LH, LH/FSH,T,FINS,HOMA-IR weresignificantly higher in PCOS group than control group(P<0.01)ï¼›but there was lowerFSH level in PCOS group than control group(P<0.05). BMI,WC,WHR,FINS,P2hINS,HOMA-IR were higher in PCOS-OW/OB subgroup than in PCOS-NW subgroup, andBMI, WC, WHR, TG, LDL, FBG, P2hBG, FINS, P2hINS, HOMA-IR were higher inControl-OW/OB subgroup than in Control-NW subgroup, while HDL was lower.Compare to Control-OW/OB subgroup, LH, LH/FSH, T, HOMA-IR were higher inPCOS-OW/OB subgroup; And compare to Control-NW subgroup, WC, LH, LH/FSH, T,FINS, P2hINS, HOMA-IR were higher in PCOS-NW subgroup.2. Comparisons of OXA and Nesfatin-1:concentrations of fasting serum OXA andNesfatin-1were significantly lower than control group(P<0.01).Both fasting OXAlevel and Nesfatin-1level were higher in Control-NW subgroup and Control-OW/OBsubgroup than correspondent subgroup of PCOS. The serum OXA levels of OW/OBsubgroup were lower than NW subgroup both in PCOS group and Control group.3. Correlation analysis in PCOS group revealed that serum OXA levels were correlatednegatively with LH, LH/FSH, FBG, P2hBG, HOMA-IR(r=-0.313ã€r=-0.302ã€r=-0.353ã€r=-0.294ã€r=-0.283, P<0.05or P<0.01),serum Nesfatin-1levels were correlatednegatively with FBGã€FINSã€HOMA-IR(r=-0.379ã€r=-0.428ã€r=-0.476,P<0.01),after adjusting the age and BMI。Conclusions1.There were significant insulin resistance and hyperandrogenemia in newly-diagnosedPCOS patients.2. Fasting serum OXA and Nesfatin-1levels decrease in patients with PCOS, but thespecific mechanism needs to be further studied, which may paly roles in thedevelopment of PCOS.. |
