The Causes Of Female Infertility And Case Study On Its Relevant Factors

[Objective]To analyze the causes of female infertility patients and its relevant factors, explore the main diseases lead to female infertility and the relevant high risk factors, to provide reference for prevention and treatment on infertility.[Subjects]Choose216female infertility patients as case group who received treatment at the Reproductive Center in One Three-Level and Class A Hospital located in Jinan from May,2013to January2014.Choose216female with normal reproduction function as matched group who received treatment at the Obstetrics and Gynecology Department in the same hospital at the same time, including natural conception pregnant,unplanned pregnant who will have abortion.[Methods]1. Adopting hospital-based1:1matched case-control study, infertility patients as case group, female with normal reproduction function as matched group.2.To get data through clinical inspection and diagnosis together with the questionnaire survey.(1)Clinical inspection and diagnosis:collect medical history, carry on system inspection and diagnosis in both groups, total8items.(2)Questionnaire survey: both groups fill in the questionnaire carefully. Total7items with126questions.3.Establish database with Epidata3.1, describe and analyze the data with SPSS16.0 software and describe the factor’s composition with composition ratio. Make univariate analysis on every factor with Conditional Logistic Regression Model and confirm the relevant single factors. Make multivariate analysis on the confirmed relevant single factor with Multivariate Conditional Logistic Regression Model(Conditions Logistic enter level=0.1, eliminate level=0.15).[Results]432valid questionnaire in the survey are awarded,216each in case group and matched group.The following are results:1.Analysis of the causes proportion:Fallopian Tubes Factor(34.3%), Ovarian Factor(23.1%), Pelvic Inflammation Factor(16.2), Immune Factor(8.3%),Unknown Factor(5.1%), the Uterus Factor(4.6%),Vaginitis factor(4.6%), Endocrine Disease Factor(2.3%), Genetic Factor(1.4%). The main two factors are Fallopian Tubes Factor(34.3%)and Ovary Factor(23.1%).Specific statistics showed that: among tubal infertility, sexually transmitted diseases can be27.05%,while pelvic nonspecific inflammation is25.7%and pelvic endometriosisis24.3%. Among ovarian oviposit obstacle, Polycystic ovary syndrome can be32.0%, while ovarian chocolate cyst is30.0%and sexually transmitted diseases is12.0%.2.Univariate analysis showed that related factors with significant difference in female infertility are:menstrual rule, the number of sexual partners, age of first sexual intercourse, abortion, ectopic pregnancy, vulva vagina washing, sexual intercourse pain, menstrual period sex, dysmenorrhea, premarital sex, endocrine disease history, auto-immune disease history, rheumatism, gynecological inflammation, thyroid disease, gastrointestinal disease,occupation,cultural degree, using mobile phone before sleep, having dessert and fried foods, stay up late, perm, having pickled food, edible cottonseed oil, drink alcohol,sauna,use electric blanket,smoking,night work,use microwave oven,colleague smoking, house decoration, poisons/heavy metals/pesticides, relationship of parent,colleague and couple,psychological pressure, helplessness, disappointment. 3. Multivariate analysis showed that age,endocrine disease history, gynecological inflammation, menstrual rule, age of first sexual intercourse, the number of sexual partners, having dessert and fried foods, stay up late, using mobile phone before sleep, poisons/heavy metals/pesticides, psychological pressure are main related factors.[Conclusions]The main causes lead to female infertility are Tubal obstruction and Ovulation disorders.2. Among Biological Factors, gynecological inflammation,EMT and PCOS are the main diseases result in Tubal obstruction and Ovulation disorders.So, they can be regarded as the risk factors of female infertility indirectly.3. Among Social Factors, age, low age of first sexual intercourse, too many sex partners, stay up late, high psychological pressure, having dessert and fried foods, using mobile pone before sleep...etc. are high risk factors of female infertility.4. Among Environmental Factors, contacting poisons, heavy metals and pesticides are high risk factors of female infertility.
