Objective: The purpose of this study is to explore the correlation between red cellvolume distribution width and the degree of atherosclerotic lesions, and the correlationbetween the prognosis of acute myocardial infarction; and red blood cell volume distributionwidth is independent of coronary atherosclerotic heart disease risk prediction factors.Method: From2012October-2013year in October during,In499cases of the first hospitalof Jilin University Bethune Department of Cardiology operation patients underwent coronaryangiography as the object of study. According to the results of coronary artery angiographywere divided into coronary heart disease (CHD) group of332cases and167case of normalcontrol group, CHD group was divided into1vessel disease,2vessel disease and multiplevessel disease group, Based on the history and the contrast of the CHD group was dividedinto stable angina pectoris (SAP) group, unstable angina pectoris (UAP) group, non STsegment elevation acute myocardial infarction (NSTEMI) group and acute ST segmentelevation myocardial infarction (STEMI) group.Result:1.comparison of general clinicaldata:(1) CHD group and the control group in gender, smoking, hypertension, diabetes, age,creatinine, FPG, NE ratio, EF, LVED, The difference was statistically significan(tP<0.05);(2)Comparison of CHD Sanya group, female, hyperlipidemia, smoking, hypertension, diabetes,age, creatinine, LVED, the difference was statistically significant (P<0.05).2.CHD groupthan in the control group, RDW was significantly increased, and the number of vascularlesions in different subgroups, RDW value increased with the number of vascular lesions(P<0.05).3.The clinical types and clinical data were compared between the4groups: theresults showed that serum creatinine, RDW, LVED in unstable angina, non ST segmentelevation myocardial infarction group and ST segment elevation myocardial infarction groupwas significantly higher than that in patients with stable angina patients; The peak value ofCTnT in STEMI group was significantly higher than the other3groups,(P<0.05), EF valueswere significantly lower than the other3groups of patients in the STEMI group(P<0.05); Incomparison with NSTEMI group and SAP group, the difference was statistically significant (P<0.05).4.Correlation analysis of Gensini score and RDW、NGP and hs-CRP among thethree display that RDW and Gensini integral was dominant linear positive correlation(r=0.184, p=0.0004), hs-CRP and NGP was significantly positive correlated with Gensiniscore (r=0.376, p<0.0001) and (r=0.243,p<0.001), RDW was positively correlated withhs-CRP and NGP (r=0.178, P<0.0001)and (r=0.199,P=0.032),.5.Acute myocardialinfarction group,RDW were bivariate correlation analysis and creatinine,BNP,hs-CRP andCTnI peak,results showed that RDW and creatinine,and BNP,hs-CRPand CTnIare positivelyrelated to the peak,and the correlation coefficients (r=0.047,p=0.023)、(r=0.104,p<0.001),(r=0.045,p<0.001) and(r=0.190,p<0.001),and negatively correlated with EF,thecorrelation coefficients(r=-0.046,p=0.002).6.Multivariate CHD regression analysis of riskfactors of logistic:the results showed that gender, age, hypertension, RDW, Hs-CRP is anindependent risk factor for coronary heart disease, in which women were protective factorsof coronary heart disease, but found no smoking, diabetes, high cholesterol, creatinine, thepeak value of CTnI as an independent risk factor for coronary heart disease.Conclusion:1.RDW and inflammatory markers was Hs-CRP and the percentage ofneutrophils, showed that chronic inflammation may be related to the RDW.2.RDW increased coronary vascular pathy count and Gensini score wassignificantly positively correlated, indicating that elevated RDW may reactcount and coronary artery disease severity.3. In acute ST segment elevation myocardial infarction group RDW was apositive correlation with the BNP and CTnI peak, and negatively correlatedwith EF values, show that higher RDW can predict the infarction area andprognosis of acute myocardial infarction。4.The risk of RDW and coronary heart disease are closely related, logisticanalysis showed that, after adjusting for other factors, RDW is an independentpredictor of coronary heart disease.
