| Objective:Three surgical approaches are applied to guarantee both complete removal ofmaxillary sinus cyst and normal function of nasal cavity. The cure rate andpostoperative complications are analyzed statistically to assess the effectiveness of thethree surgical approaches.Method:This study was conducted between January2008and January2012at the Depart-ment of Otorhinolaryngology, the Second Hospital attached to Jilin University(Changchun, Jilin, China). A total of160patients (83males,77females; aged from16to56years, mean age33.83) with maxillary sinus cyst were enrolled in this study.The course of disease is between20days to10years. Paients complaint mainly aboutfacial fullness and numbness in the cheek, headache, intermittent yellow watery nasaldischarge. All the160patients underwent preoperative CT scan, then the locations ofnasal cavity cyst are confirmed. Among them.23of them had nasosinusitis andpolyposis besides cysts in the maxillary sinus.10of them had simple nasosinusitis.7patients had septal deviation. There were inferior turbinate hypotrophies to varyingdegrees in11subjects.45cysts occurred in the bottom wall,52in the posterior andlateral wall.34cysts were located in the medial wall of maxillary sinus, and29in theanterior and inferior wall. All lesions were confirmed to be mucous cyst bypostoperative pathology and were divided into three groups.54patients underwentendoscopic resection with a inferior nasal meatus mucosal flap.62subjects weretreated via natural sinus ostia of maxillary sinus. And44operations were performedvia canine fossa puncture assisted by nasal endoscope.Result:All160patients underwent nasal endoscopic surgery, and the postoperativepathology results were mucosal cyst.The follow up ranged from6months to2years.Reviews including CT scan and endoscopy were conducted at the outpatient clinic. Theanalysis of cure rate and complications were accomplished using SPSS19.0statistics software and under the evaluation criteria made by the Chinese OtolaryngologyResearch Team. There is no statistical difference between the three groups(P>0.05). Asfor complications, there is significant difference between the three groups in facial androot numbnessã€facial swelling,and nasal cavity adhesion (P<0.05).However, there isno significant difference in epiphoraã€Maxillary sinus empyema (P>0.05).Conclusion:Statistical analysis confirmed that there is no significant difference in the cure rate.But the difference is distinctive in the occurrence of complications. Compared withthe operations via natural sinus ostia of maxillary sinus,endoscopic resection with ainferior nasal meatus mucosal flap and operations via canine fossa puncture assistedby nasal endoscope can remove cysts completely and maintain the integrity of themiddle nasal meatus.therefore, the latter two methods are worth applying widely. |
