Analysis Of Factors In Long-acting Contraceptive Measures And Effects Of Application Of Hebei Province

Objective:The application of long-acting contraceptive measures status and trends of married couplesat childbearing age in Hebei province can be understood through the survey.It can be better forthe people of childbearing age to provide safe, appropriate, effective contraceptive servicesthrough analyzing of the influencing factor. The government can purchase intrauterine devicewhich has high effective rate, less side effects and high acceptance rate by comparing use effectindex between government procurement directory inside and outside each type intrauterinedevice. It can be also provide a reliable reference of the contraceptive technology research anddevelopment of new products in the12th Five Year Plan period.Method：We select33provincial family planning devices with adverse reactions fixed monitoringsites as the sample points in Hebei Province. All the new married couples of child-bearing agewho take long-acting contraceptive measures From October1,2011to September30,2013werechosen as samples of investigation. Stratified sampling method was used in these targetpopulation and it was a2years of follow-up. We know about the information that related theapplication of long-acting contraceptive measures, such as demographic characteristics,long-term product information, operation situation and the departments/personnel postoperativefollow-up etc. Data analysis using SPSS19.0.To describe the long-acting contraception information ofinvestigation objects, and count data used in forming the ratio (%), using χ2test for single factoranalysis and multi-factor non-conditional Logistic regression model for multivariate analysis.Correlation analysis comes out by the spearman rank correlation.Result：This survey has collected information on13082cases. In these samples, the insertion ofIUD was used in12775cases, accounting for97.65%;287women were taken thesterilization,accounting for2.19%, male sterilization were found in13cases, accounting for0.1%;7cases has the subcutaneous implants, accounting for0.05%. Operation for long-actingcontraceptive measures has related to the correlation with age, the number of parity, children andother demographic factors, it has nothing to do with the way of last birth time. The institutionwho take responsible for the contraceptive measures were mainly in rural and family planningtechnical service department, accounting for77.01%and76.54%. All the people in the provincehad high degree of informed consent; in survey, there were81.94%people who chose by themselves,18.04%were under the guidance of passive choice; and0.02%were passive choose.People take operation for long-acting contraceptive measures mainly in20-29years old（75.11%）and rural towns are more than countries. IUD application have varieties kinds, thereare differences in urban and rural areas on using them. the large use number of intrauterinedevice in the government internal procurement including TCu220c (common type) accountingfor26.98%, Yuan Gong220IUD accounting for22.32%, MCu375IUD accounting for9.08%,the large use number of intrauterine device in the government external procurement includingJeanne ring, accounting for5.34%, Anshu ring,accounting for3.87%, Mcu IUD accounting for1.61%The devices with one-year high continuation rates in government procurement catalogwere MCu375and MCU IUD; the top5adverse reaction including menorrhagia,accounting for2.04%, lower back pain accounting for1.72%,abdominal pain accounting for1.48%,menostaxisaccounting for1.10%,irregular bleeding accounting for0.72%. Age, the last reproductive wayand the operation time are correlative with the adverse reactions of IUD. Multiple factorsanalysis shows that the reproductive way and the operation time are protect factors of the adversereactions of IUD. Recipient age, parity and the amount of child was positively correlated withadverse reaction, but adverse reaction has little association with the operation time. The reasonfor not sustained lien of intrauterine device in proper order were" drop"(32.24%),"others"(28.95%),“inflammation”(22.37%),“pregnancy"(11.84%),“IUD adverse discontinuation rate”0.79%.Conclusion:1. The use of IUD in long-term contraceptive measures were in the largest share in Hebeiprovince. The male sterilization, the female sterilization and subcutaneous implants were lessused respectively in long-term contraceptive measures.2. Women of childbearing age has a high rate of contraceptive informed in Hebei and themajority of people choose for birth control measures independently.3. The institution who take heavy responsible for the contraceptive measures were mainly inrural and family planning technical service department respectively.4. Age, the last reproductive way and the operation time are correlative with the adversereactions of IUD.5. The intrauterine device use tends to be rationalization and diversification, and the lossrate, pregnancy rates with device and adverse reactions rate are low.
