Objective Comparison of laparoscopic cholecystectomy (LC) and open cholecystectomy (OC) on stress index、the immune function and liver function of the patients before and after operation, provide a reference basis for selection of surgical operation of the patients with gallbladder stones.Mthods From2010February to2013February in Inner Mongolia Baogang Hospital Department of general surgery in laparoscopic cholecystectomy (group LC) and open cholecystectomy (OC group) of the200cases of patients, the detection of CD3+, CD4+, CD8+was used by flow cytometry,plasma cortisol was measured by radioimmunoassay (Cor) concentration, the detection of C-reactive protein (CRP), epinephrine, gamma glutamyl transpeptidase (GGT), alanine aminotransferase (ALT), serum total bilirubin (TBIL) and aspartate aminotransferase (AST) was tested by enzyme linked immunosorbent assay, using rate nephelometry method for the determination of immunoglobulin A (IgA), immunoglobulin G (IgG), immunoglobulin M (IgM),blood glucose by Germany Roche glucose meter detection. Results1. The two groups before operation in patients with serum CRP, epinephrine, cortisol (Cor) concentration, blood glucose (BG) were not statistically significant (P >0.05), after Id stress indexes and preoperative ratio are increased, the difference was statistically significant (P<0.05), after all the7d index level was lower than after Id, and the LC group decreased faster, compared with OC group was statistically significant (P<0.05);2. Operation before the difference of each index IgA, IgG, IgM, CD3+, CD4+, CD8+in the serum of the patients of two groups were not statistically significant (P>0.05),1d after operation, CD4+decreased, CD8+increased, the immune function indexes and preoperative ratio were decreased, the difference was statistically significant (P<0.05). After7d and CD4+began to rise, CD8+gradually returned to normal, the rest of the index began to rise, close to the preoperative level, and faster recovery LC group, the difference was statistically significant (P<0.05);3. Operation before the difference of each index of two groups of patients with serum GGT, ALT, TBIL, AST and liver function were not statistically significant (P>0.05),1d after operation,7d group and control group and each index before operation, the difference was statistically significant (P<0.05), after all the7d index level was lower than Id after the operation, the difference was statistically significant (P<0.05). Conclusion1. LC and OC on immune functions, the preoperative immune function and liver function tests were normal liver function have a certain impact, but only for a change, does not affect the patient to resume;2. Laparoscopic cholecystectomy less pessimal stimulation to the body, can become a security operation in treatment of cholecystolithiasis, have broad application prospects.
