| Objective:To observe the clinical effection of the auricular acupuncture on inguinal hernia surgery general anesthesia with propofol and remifentanil total intravenous anesthesia.by target-controlled infusion techniques (TCI)Method:50cases of elective hernia surgery patients,ASA I or Ⅱ,were randomly divided into the ear acupuncture(EA) group and the control group n=25cases.control group:propofol+remifentanil;EA group:EA(homolateral FuGuGou acupoint, GanZang, PiZhiXia, ShenMen acupoint.)plus control group.Blood pressure, Saturation of pulse oxygen (Sp02),electro-cardiogram and bispectrial index(BIS) were continuously monitored after patients entered operation room. EA group,Pressure method used for the oppression of tape affixed to the ear acupoint.Pressed under ten per acupoint.30min Induction time,underwent intravenous anesthesia.EA were conducted for each patients in group EA usque ad surgery finished.The profofol was started a predict blood concentration of1.5ug/L delivered by Diprifusor,and increased by0.3ug/L every30s until patient lost their consciousness. After that,effect-site concentration of propofol kept constant and infused remifentanil a predict blood concentration of2.0ug/L and increased by0.3ug/L every30s until patient lost response to a standard noxious stimulus.Bispectral index,the plasma and effect-site concentration, changes in perioperative hemodynamics was measured when the pain disappeared.Results:1. The differences of gender, age, weight, height were not statistically significant in two groups.2. Compared with the control group, ear acupuncture group induction process more stable hemodynamics(p<0.05), the heart rate and blood pressure control group extubation higher (p>0.05)3.Lost consciousness,ear acupuncture group propofol plasma effect-site concentration was significantly lower than the control group, the difference was statistically significant(p<0.05) 4.When the pain response disappeared,ear acupuncture group remifentanil plasma and effect-site concentration was significantly lower than the control group, the difference was statistically significant(p<0.05)5.Bury the needle ear, the ear acupuncture group and the control group BIS values were87±9,96±1,the difference was statistically significant (p<0.05),described BIS and ear buried the needle has a good correlation.6. Compared with the control group,ear acupuncture group remifentanil during surgery on the total dose was significantly lower in the control group a total dose of propofol drug did not change significantly during the procedure.Conclusion:1.When consciousness and pain,ear acupuncture during TCI induced decrease of propofol and remifentanil plasma and effect-site concentration.2.During perianesthesia,ear acupuncture reduces the total dosage throughout the anesthesia with propofol/remifentanil.3.During general anesthesia, combined with hemodynamic changes, BIS and depth of anesthesia cases a good correlation. |
